Study On Liposomal Paclitaxel/Gemcitabine Combined Platinum As First-line Chemotherapy For Locally Advanced And Metastatic Squamous Cell Lung Cancer

Abstract/Summary:
Objective:In this study,analysis of the recent and long-term efficacy of liposomal paclitaxel or gemcitabine combined platinum for locally advanced and metastatic lung squamous carcinoma,as so as adverse reaction and the status of the survival benefit,then to explore the efficacy and tolerability of more suitable for locally advanced and metastatic patients with lung squamous carcinoma.Methods:174 cases which were collected from January 2009 to October 2015 in The First Affiliated Hospital of Nanchang University confirmed by pathology or cytology of patients with stage IIIA ~ IV lung squamous carcinoma,respectively received chemotherapy with liposomal paclitaxel combined platinum or gemcitabine combined platinum(cisplatin/nedaplain/carboplatin).The objective response rate(ORR),disease control rate(DCR),progression free survival(PFS)and overall survival(OS)in two therapies are evaluated by the Response Evaluation Criteria in Solid Tumors(RECIST).The adverse reactions is assessed using the National Cancer Institute Common Toxicity Criteria(NCI~CTC 3.0).The SPSS 21.0 were used to data analyses.The test level is defined as P<0.05 and the difference is statistically significant.Results:There are 86 cases of patients were collected in liposomal paclitaxel group(group L)and 88 cases of patients were collected in gemcitabine group(group G).In two groups,ORR was 51.2% and 40.9% respectively(P=0.224)and DCR was 82.6% and 85.2% respectively(P=0.684),the ORR and DCR are have no statistical difference.The PFS is 9.8 months in group L and 8.2 months in group G(P=0.007),and the difference was statistically significant.The OS is 15.4 months and 14.7 months(P=0.687),respectively,there was no statistically significant difference.In the aspect of adverse reactions,the overall incidence of leukopenia(P = 0.004),thrombocytopenia(P = 0.000),nausea and vomiting(P = 0.000),hair loss(P = 0.012)of L group was lower than those of group G,difference is statistically significant.But the incidence of grade III-IV,only thrombocytopenia(P = 0.013)and nausea/vomiting(P = 0.048)have significant difference.The incidence of anemia,liver and kidney impairment,allergy of two groups have no significant difference.Conclusion:In the first-line chemotherapy of locally advanced and metastatic lung squamous carcinoma,liposomal paclitaxel combined platinum has similar recent effect with gemcitabine combined platinum,which can significantly prolong the PFS,and weaken the toxicity of thrombocytopenia,as the same as nausea/vomiting.Liposomal paclitaxel combined platinum may be superior to gemcitabine combined platinum for locally advanced and metastatic squamous cell lung cancer.
