Objective:To explore the related factors of Rheumatoid arthritis(RA)associated with interstitial lung disease(ILD).Method:349 patients with RA treated by rheumatology department of Jiangxi Provincial People's Hospital from June 2013 to February 2016 were divided into RA group and RA combined with ILD group(RA-ILD group)according to high-resolution CT(HRCT),the general data?related laboratory test and pulmonary function indicators were compared between these two groups.Result:Among these 349 newly diagnosed RA patients,ILD was identified in 69patients(19.77%).The main respiratory symptoms of RA-ILD patients were exertional dry cough and dyspnea,and the main abnormal performance in HRCT were interlobular septal thickening ? reticular and nodular shadows.There was significant difference in both of the two groups of gender(P<0.05);the mean age,smoking rate ? DAS28 ? RF,anti-CCP antibody,GPI of RA-ILD group were significantly higher than those of simple RA group(P<0.05);and there was no significant difference in course of disease and test of erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),fibrinogen,D-dimer and immunoglobulin(IgG,IgA,IgM)between the two groups(P> 0.05).68 patients(37 patients in RA group and 31 patients in RA-ILD group)underwent pulmonary function tests,the DLCO?VC?FVC?FEV1.0 of RA-ILD group were significantly lower than those of simple RA group,the differences were statistically significant(P<0.05);the incidence of ventilation dysfunction and diffuse dysfunction in RA-ILD group were significantly higher than those of control group(P<0.05);and there were no significant difference in incidence of small airway dysfunction and FEV1/FVC between the RA-ILD group and simple RA group.The logistic regression analysis showed that age,GPI andDAS28 were closely related to RA-ILD(P<0.05);and the correlation analysis showed that DLCO had no correlation with age,DAS28 and GPI in RA-ILD group(P> 0.05).Conclusion:ILD is a common complication of RA patients,especially in the elderly,expression of high titer GPI and high disease activity of RA patients prone to concurrent ILD.Pulmonary function tests of ventilation dysfunction,diffuse dysfunction in patients with RA-ILD is more significant.
