Objective: To determine the serum Ig E,25(OH)D,T lymphocyte subsets and B lymphocyte and the expression of partial CD28 family receptors on CD4+T cell in pediatric asthma,and explore the correlation with asthma,speculate its role in the pathogenesis of asthma,provide further proof for clinical prevention and treatment of asthma.Methods: Select 16 cases of preschoolers with asthma in the First Affiliated Hospital of Wannan Medical College during one and a half years,13 cases of preschoolers with general bacterial infection and 13 cases of preschoolers for health examinations were detected as control group in the same period.There were little significant discrepancies in weight,age and gender among the control group,the infection group and the asthma group(P>0.05).The diagnosis of asthma accord with Zhu Futang Practice of Pediatrics8 th Edition.The infection indexes in preschoolers with general bacterial infection were abnormal.There were no other complications in these children.Children with asthma and general bacterial infection were not treated prior to admission.2.0-2.5ml fasting venous blood was taken from individual preschooler within 24 hours after admission,and separately maintained in the anticoagulant tube containing EDTA.Ig E was tested by turbidimetry.25(OH)D was determined by radioimmunoassay.The percentage of T lymphocyte subsets and B lymphocyte and the expression of CD28,CTLA-4 and BTLA on CD4+ T cells were analyzed by flow cytometry.Results: 1.There were little significant discrepancies in 25(OH)D among the three groups(P > 0.05).2.Ig E in the asthma group was significantly higher than that in the control group and the infection group(P < 0.01).There was no significant discrepancies in Ig E between the infection group and the control group(P > 0.05).3.The percentage of T lymphocyte subsets and B lymphocyte among the three groups was statistically significant:(1)CD3+ T cells?CD3+CD8+ T cells in the asthma group and the infection group were decreased dramatically compared with those in the control group(P < 0.01);there were little discrepancies in CD3+ T cells and CD3+CD8+ T cells between the asthma group and the infection group(P > 0.05).(2)CD3+CD4+ T cells in the asthma group was increased dramatically compared with that in the infection group and the control group(P < 0.05).There was little difference in CD3+CD4+ T cells between the infection group and the control group(P > 0.05).(3)The ratio of CD4+/CD8+ in the asthma group was increased dramatically than that in the infection group and the control group(P < 0.01).The ratio of CD4+/CD8+ in the infection group was also increased dramatically compared with that in the control group(P < 0.01).(4)CD3-CD19+ B cells in the asthma group was increased dramatically compared with that in the infection group and the control group(P < 0.01).There was little difference in CD3-CD19+ B cells between the infection group and the control group(P > 0.05).4.The expression of CD28 on CD4+ T cell in the asthma group and the infection group were dramatically higher than that in the control group(P < 0.01);there was little difference in CD4+CD28+ T cells between the asthma group and the infection group(P > 0.05).5.The expression of BTLA on CD4+ T cells in the asthma group was dramatically higher than that in the control group(P < 0.01)and the infection group(P < 0.05).There was little difference in CD4+BTLA+ T cells between the infection group and the control group(P > 0.05).6.CTLA-4 on CD4+ T cells was not detected in the control group,the infection group and the asthma group.Conclusions: Costimulatory molecules are involved in the pathogenesis of asthma:(1)CD28 is up-regulated on CD4+T cells in children with asthma and participates in the immunoregulation of asthma by promoting T cells activation and establishing an effective immune response;(2)The expression of CTLA-4 on CD4+ T cells was not determined among the three groups,but it cannot be denied that they are involved in the pathogenesis of asthma,which may be related to failing to up-regulate for lacking inhibiting signal in asthmatic children.CTLA-4 mediates negative signal transmission;(3)The expression of BTLA on CD4+ T cells is up-regulated and participates the negative regulation of immune response by inhibiting the activation of T cells in asthmatic children.
