Objective To explore the pulmonary protective effect of ulinastatin during cardiopulmonary bypass(CPB).Methods The randomized controlled trials(RCTs)about exploring the pulmonary protective effect of ulinastatin during CPB were collected by searching CQVIP,CECDB,CNKI,CBMdisc before March 2017.RevMan 5.3was used to evaluate the included studies strictly and analyze extracted datas.Results 18 RCTs were included,involving 812 patients.The patients received ulinastatin in experimental group,and those who received blank control were chosen as control group.At 1 h after CPB in experimental group compared with blank control group,the concentration of TNF-? was significantly decreased [SMD=-2.64,95%CI(-3.23,-2.06),P< 0.01],the concentration of IL-6 was decreased [SMD=-2.84,95%CI(-3.77,-1.91),P< 0.01],the concentration of IL-8 was decreased [SMD=-3.15,95%CI(-4.08,-2.23),P<0.01],while the concentration of IL-10 was increased [SMD=1.47,95%CI(1.17,1.77),P>0.01],also RI was decreased [SMD=-1.81,95%CI(-2.61,-1.01),P< 0.01].Conclusion Ulinastatin can significantly reduce the postoperative inflammatory response in CPB patients and has better effects in pulmonary protection than blank control group in a period of time.
