The Study Of Sonographic Differential Diagnosis Between Intrauterine Pregnancy And Cesarean Scar Pregnancy In The First Trimester

Objective:To investigate the value of ultrasound for the differential diagnosis of normal intrauterine pregnancy(IUP)versus cesarean scar pregnancy(CSP).Methods:We analyzed 222 ultrasound images between 5 and 10 weeks of gestation in our hospital from January 2015 to January 2017: 74 cases of CSP as the research group,at the same time,148 cases of IUP(74 cases of Intrauterine pregnancy-None cesarean delivery(IUP-NCD)and 74 cases of Intrauterine pregnancy-Cesarean delivery(IUP-CD))as the control group,the distance to the center or the average center of the gestational sac(GS)relative to the mediatrice of the corpus uteri and The thickness of anterior lower uterine segment myometrium was measured.Results:1.The distance to the center or the average centerof the GS relative to the mediatrice of the corpus uteri was statistically signifcant difference between the IUP group and the CSP group(P<0.05),the IUP-NCD group andthe IUP-CD group were no significant difference(P>0.05),the IUP-NCD group and The IUP-CD group were compared with the CSP group was statistically significant difference(P < 0.05).2.There was no significant difference of the distance to the center of GS relative to the mediatrice of the corpus uteri and the distance to the average center of GS relative to the mediatrice of the corpus uteri(P>0.05).3.There was no significant difference of the distance to the center or the average center of GS relative to the mediatrice of the corpus uteri and surgical pathology diagnosis(P>0.05).4.There was no significant difference of the distance to the center or the average center of GS relative to the mediatrice of the corpus uteri in the differential diagnosis of the endogenous type of CSP and the exogenous type of CSP.5.The thickness of anterior lower uterine segment myometrium was statistically signifcant difference between the IUP group and The CSP group(P<0.05),the IUP-NCD group?the IUP-CD group and the CSP group were statistically significant(P<0.05),the endogenous type of CSP group and the exogenous type of CSP group were no significant difference(P>0.05).Conclusion:1.The application of the distance to the center or the average center of the GS relative to the mediatrice of the corpus uteri can be used as an easy method for sonographic differentiation of IUP and CSP in the first trimester,but it is little value in the differentiation of the endogenous type of CSP and the exogenous type of CSP.2.The distance to the center or the average center of the GS relative to the mediatrice of the corpus uteri is no significant difference,One of method for sonographic is applied to the auxiliary diagnosis of CSP has high coincidence rate.3.The thickness of anterior lower uterine segment myometrium can be used as one of the indexes in the differentiation of IUP and CSP,but it is little value in the differentiation of the endogenous type of CSP and the exogenous type of CSP.
