Objective: explore the clinical application and reduce the complications of operation in intraocular silicon oil removal.Methods: clinical records of 54 patients (54 eyes) enrolled in the First affiliated hospital of guangxi medical university eye ward from January 2014 to January 2015 underwent removal of silicon oil with two incisions with 23-gauge vitrectomy system ( 30 patients 30 eyes) (23G) or 20-gauge vitrectomy system(24 patients 24 eyes) and 10 ml syringe hand control the negative pressure remove silicone oil (20G) were reviewed. The total operation time and silicone oil removal time and the diference between preoperative and postoperative IOP levels. The BCVA before surgery and postoperative Id, 1w, 3mo after surgery;and the eye comfort postoperative 1d, 3d 1w, 1mo after surgery;rate of retinalreattachment and silicone oil residue also compared.Results: the total operation time was 27.44±3.5min (23G group) versus 26.74±3.7min(20G group),(t=0.708 p=0.482).The silicone oil removal time was 22.1±2.05 min (23Ggroup) versus 17.33±2.5min (20G group),The difference of the silicone oil removal time was significant. The difference of BCVA before and after surgery between two groups was ont significant. The difference of IOP before and after surgery between two groups was ont significant,but the postoperative IOP was lower than the preoperative in every group.There were 9 eyes (37.5%) appeared lower IOP (<10mmHg) 1d after operation, 4 eyes(16.7%)appeared lower IOP ( < 10mmHg) 1W after operation, the patients recovered 1mo after operation in 20G group .There were 3 eyes (10% ) appeared lower IOP ( < 10mmHg) 1d after operation , the patients recovered 1w after operation in 23G group.The symptoms such as photophobia, tears, the eye looked reddish,dry eye symptom within a month after' operation in 23G group was obviously lighter ,the two groups had no obvious discomfort after 1mo.There were leye(3.33%) experienced retinal redetachment after operation in 23G group and 4 eyes (16.67% ) in 20G group,but the difference was not significant (p=0.093>0.05).Only 2eyes with a little silicone oil residue in the two groups, which affected the life and not obvious absorption in the process of follow-up in the 20G group.Conclusion: the two incisions silicone oil removal with a 23-gauge (23G)vitrectomy system with the small sclera, incisions and linear control pressure, has the advantages of mild postoperative reaction a postoperative reaction, rapid recovery, fewer complications.
