Objective: To evaluate changes of left ventricular systolic function in patients with acute anterior ST-segment elevation myocardial infarction(STEMI)12 months after percutaneous coronary intervention(PCI)by three-dimensional speckle tracking echocardiography(3D-STE).Material and method: 41 healthy persons(control group)and 41 patients(cases)with STEMI Post-PCI 12 months were examined by conventional echocardiography and 3D-STE.Left atrium diameter(LAd),left ventricular end-diastole diameter(LVDd),left ventricular end-diastole volume(LVEDV),left ventricular end-systole volume(LVESV)were measured and left ventricular ejection fraction(LVEF)were calculated by conventional echocardiography.Global longitudinal strain(GLS),global radial strain(GRS),global circumferential strain(GCS),global area strain(GAS),left ventricular ejection fraction(LVEF),and each segment longitudinal strain(LS),radial strain(RS),circumferential strain(CS)and area strain(AS)were examined by 3D-STE.Result: 1.Conventional echocardiographic parameters :LAD,LVDd,EDV,ESV were increased in patients with STEMI Post-PCI 12 months than that in control group significantly(P < 0.01).2.Left ventricular global strain value : Global systolic function was decreased in patients with STEMI Post-PCI 12 months,GLS,GRS,GCS,GAS and LVEF were deduced(P < 0.01).3.Left ventricular local strain value: the absolute value of basal,middle and apical of the anterior wall,anterior septum basal,middle and apical septum,apical inferior wall and apical lateral wall LS,RS,CS,and AS in patients with acute myocardial infarction caused by the left anterior descending branch lesions were decreased than that in control group(P<0.05),the longitudinal strain value of the section(LS)7/16,circumferential strain value of the section(CS)8/16,radial strain values(RS)7/16,area strain value(AS)8/16 were reduced.4.Correlation analysis: GLS,GRS,GCS,and GAS was significantly positively related to LVEF,the correlation coefficient respectively r = 0.843?P < 0.0001;r = 0.838?P < 0.0001;r = 0.837?P < 0.0001;r = 0.880?P < 0.0001.Conclusion: 1.Left Ventricular Systolic Function in Patients with STEMI after PCI procedure 12 months was reduced,the strain values include global longitudinal strain(GLS),global radial strain(GRS),global circumferential strain(GCS)and global area strain(GAS)were decreased.2.The systolic function of corresponding segments was decreased in patients with STEMI due to myocardial ischemia,the strain values in patients after PCI procedure 12 months include longitudinal strain(LS),radial strain(RS),circumferential strain(CS)and area strain(AS)were reduced.3.Myocardial deformation ability were decreased in patients with STEMI,although PCI can improve systolic function,the whole left ventricular systolic function was still lower than normal people,long-term follow-up is necessary after PCI for 12 months.
