Clinical Observation On Acupuncture Treatment Of Residual Oizziness After Manipulative Reduction For Benign Paroxysmal Positional Vertigo

Objective:To evaluate the clinical efficacy of acupuncture for the treatment of residual dizziness affer manipulative reduction for benign paroxysmal position vertigo.Methods:A total of 60 patients that sutfered from residual dizziness after receiving successful tceatment of Epley's maneuver for posterior semicircular canal benign paroxysmal positional vertigo(PC-BPPV),in the second Affiliated Hospital of Heilongjiang University of TCM from septembcr 2015 to danuary 2017,were randomly divided into three group according to the method of random number table.The acupuncture group,was treated by acupuncture once a day,30 min each time,Made 6days a treatment course,and had one day off affer that,thetotal is 2 courses.And the main acupoints were the Yunting area,Taiyang(EX-HN5)and Fengchi(GB20).The western medicine groupwas treated by betahistine mesylate,12mg each time,tid.po.Made 7 days a course and two courses were needed.And the control group received only manipulative reduction.20 cases were put in each group.By analysing the results of five scales(DHI,TCMSSS,the signs and symptoms of vertigo severity scale,SAS and SDS)fulfilled before treatment and one week,2weeks 4weeks,8weeks affer treatment,compared the clinical efficacy of the three groups.Results:(1)DHI scores:While comparing the scores after with before receiving treatment of the acupuncture group,the differences existed,and of statistical difference(P<0.05).And compared with western medicine and control group respectively,acupuncture group got better scores,the differences were statistically significant(P<0.05).As for western medicine and control group,compared with the scores before treatment,that 2 weeks,4 weeks,8 weeks after treatment were statistically significantly better(P<0.05).Had comprison between western medicine and control group,the differences were statistically significant(P<0.05).(2)The Signs and Symptoms of Vertigo Severity Scale:While comparing the scores after with before receiving treatment of the acupuncture group,the differences existed,and of statistical difference(P<0.05).And compared with western medicine and control group respectively,acupuncture group got better scores,the differences were statistically significant(P<0.05).As for western medicine and control group,compared with the scores before treatment,that 4 weeks,8 weeks after treatment were statistically significantly better(P<0.05).Had comprison between western medicine and control group,the differences were statistically significant(P<0.05).(3)TCMSSS:While comparing the scores after with before receiving treatment of the acupuncture group,the differences existed,and of statistical difference(P<0.05).And compared with western medicine and control group respectively,acupuncture group got better scores,the differences were statistically significant(P<0.05).As for western medicine and control group,compared with the scores before treatment,that 2 weeks,4 weeks,8 weeks after the scores after with before receiving treatment of the acupuncture group,the differences existed,and of statistical difference(P<0.05).And compared with western medicine and control group respectively,acupuncture group got better scores 1 week,2 weeks,4 weeks after treatment,the differences were statistically significant(P<0.05).As for western medicine group,compared with the scores before treatment,that 2 weeks,4 weeks,8 weeks after treatment were statistically significantly better(P<0.05).And for control group,compared with the scores before treatment,that 4 weeks,8 weeks after treatment were statistically significantly better(P<0.05).Had comprison between western medicine and control group,the differences were statistically significant that 2 weeks,4 weeks,8 weeks after treatment(P<0.05).(5)Total effect:All the differences of the 3 groups before and after treatment were statistically significant(P<0.05).The total effective rate of acupuncture,western medicine and control group were 90.00%,75.00%and 60.00%.Compared with western medicine and control group,acupuncture group had better scores,significant differences existed(P<0.05).Compared with control group,western medicine group had better scores,significant differences existed(P<0.05).conclusion:1,Should be early intervention PC-BPPV by manual reset after the residual dizziness,both acupuncture and the Western medicine betahistine mesylate have obuious curative effect on the treatment of residual dizzines balance disorders and other symptoms after manipulative reduction for PC-BPPV.But the acupuncture group gets more obvious short-term effect and stabler long-term effect.2,Acupuncture has regulating effect on the treatment of mild anxiety and depression of residual dizziness after manipulative reduction for PC-BPPV.Although the effectiveness of betahistine meyylate has significant differences,the self-limitation cannot be excluded.To reduce mild anxiety and depression,acupuncture has rapid effective so that the patients could be relieved earlier.3,Compared with betahistine mesylate,acupuncture is more convenient,has no side effects.It could shorten the treatment cycle,ease the patients' symptoms earlier and improve the quality of theor tofe.
