Objective: To investigate the relationship between serum level of cystatin C and chronic atrial fibrillation as well as its clinical classification.Method :From September 2013 to November 2016,278 cases of the first hospital of Jilin University Bethune were chosen as the object of this study.The subjects were divided into 182 patients with Af and 96 patients in the control group.The atrial fibrillation group was divided into paroxysmal Af group,pesistent Af group and permanent Af group according to the European atrial fibrillation management guidelines and the North America pacemaker and the society of electrophysiology(NASPE)arrhythmia group organized the categorization of atrial fibrillation.Results:(1)Compared with the control group,the level of the serum cystat in C was significantly higher in the Af group,the difference was statistically significant(P <0.001);There was significant difference in cystatin C level in patients with different types of atrial fibrillation(P <0.001).when compared the cystatin C level,Persistent Af is higher than persistent atrial fibrillation,persistent atrial fibrillation is higher than paroxysmal atrial fibrillation,the difference was statistically significant(P <0.001).(2)Analytic result of relevance between cystatin C level and related indexes : As for patients with Af,Cystatin C level is directly positive correlation to WBC level(r=0.211,p=0.004)),percentage of neutrophile granulocyte(r=0.184,p=0.013),CRP(r=0.269,p<0.001),homocysteine(r=0.424,p<0.001)and left atrial diameter(r=0.269,p<0.001),respectively;while cystatin C level is not correlated to age,the concentration of Ca2+,Neutrophil granulocyte,the left ventricular diameter(p>0.05).(3)The levels of left ventricular diameter,left atrial diameter,neutrophil percentage,low density lipoprotein cholesterol,CRP and homocysteine in the Af group were significantly higher than those in the control group(p <0.05).The left ventricular ejection fraction was lower than that of the control group(p<0.001),the difference was statistically significant.(4)The difference of left ventricular ejection fraction,left atrial diameter,neutrophil percentage,LDL-C,CRP and homocysteine levels in patients with different types of atrial fibrillation were statistically significant(p <0.05),and the percentage of neutrophils was significantly higher in patient s with permanent atrial fibrillation than in paroxysmal Af(p <0.05).For patients with low density lipoprotein cholesterol,permanent atrial fibrillati on patients were higher than paroxysmal Af and persistent Af,the differe nce was statistically significant(P < 0.05),and left ventricular ejection fra ction,permanent atrial fibrillation patients were less than paroxysmal Af and persistent Af,the difference was statistically significant(P < 0.05).when compared the left atrial diameter,CRP and homocysteine level,the patients with permanent Af is higher than persistent Af,while those with persistent Af is higher than paroxysmal Af,the difference was statisti cally significant(p<0.05).Conclusion:1.Serum cystatin C level in atrial fibrillation group is significantly higher than the control group,there are differences between different atrial fibrillation clinical classification,its level increased with duration of atrial fibrillation.2.Serum cystatin C level and inflammatory markers CRP,WBC and neutrophilic granulocyte percentage were positively correlated,indicating that serum cystatin C is associated with chronic inflammation,involved in the occurrence of atrial fibrillation,,maintain and recurrence.3.Logistic analysis showed that the serum cystatin C level could be used as an independent predictor of atrial fibrillation when other factors were corrected.
