Objective:Through the clinical data regression analysis of a total of 156 senior hip fracture patients with total hip arthroplasty,to investigate the relevant factors for preoperative complication aggravation,or postoperative complications such as anemia,hypoalbuminemia,water and electrolyte disturbance,uroschesis and gastrointestinal reaction,and to provide reference for clinical practice.Method:Through the regression analysis among the senior hip fracture patients(?60)with total hip arthroplasty hospitalized at China-Japan union hospital affiliated Jilin University based on their clinical information,a total of 156 cases were included in the research group.The case number among these patients was recorded according to age,gender,fracture type,anesthesia method,hospital stay before operation,preoperative combined internal diseases,case number and the aggravated numbers,as well as the numbers of postoperative complications,such as anemia,hypoalbuminemia,water and electrolyte disturbance,uroschesis and gastrointestinal reaction.SPSS 21.0 and Exce1 2016 were applied to the correlation analysis regarding gender,age,anesthesia method,fracture type,hospital stay before operation and THA postoperative complication(preoperative combined internal disease aggravation and postoperative complication included anemia,hypoalbuminemia,water and electrolyte disturbance,uroschesis and gastrointestinal reaction).Measurement data was displayed as (?)x±s,and enumeration data was displayed as rate or constituent ratio(%).?~2-test was utilized to enumeration data,and statistical significance was confirmed when p<0.05.Result:1.In the 156 cases,there were 61 male cases(39.1%)and 95 female cases(60.9%);111 cases of 60-79y(71.2%)and 45 cases of ?80y(28.8%);125 cases of femur neck fracture(80.1%)and 31 cases of intetrochanteric fracture(19.9%);109 cases of lumbar anesthesia(69.9%)and 47 cases of general anesthesia(30.1%);85 cases of receiving surgery within 3 days after hospitalization(54.5%)and 71 cases of receiving surgery ?4d(45.5%).2.Preoperative combined internal disease and postoperative aggravation condition: 64 cases of preoperative combining hypertension(41.0%),17 cases of elevated blood pressure(26.6%);89 cases of heart disease(57.1%,including abnormal ECG,myocardial ischemia,CHD,MI and arrhythmia),12 cases of postoperatie aggravation(13.5%);54 cases of diabetes(34.6%)and 5 cases of elevated blood glucose(9.3%);92 cases of respiratory tract infection(58.9%)and 17 cases of postoperatie aggravation(18.5%);22 cases of urinary tract infection(14.1%)and 8 cases of increased urine WBC(36.3%);48 cases of abnormal hepatic function(30.8%)and 15 cases of elevated ALT(31.3%);19 cases of abnormal renal function(15.8%)and 3 cases of elevated serum creatinine(57.1%).3.96 cases of postoperative anemia(61.5%)and 63 cases of hypoalbuminemia(40.4%);47 cases of water and electrolyte disturbance(30.1%)and 28 cases of uroschesis(17.9%);35 cases of gastrointestinal reaction(22.4%).4.Statistical analysis results: correlation analysis of postoperative complication risk factors indicated correlation between occurrence with age,hospital stay before operation and anesthesia method(p<0.05),including obvious statistical significance with age(p<0.01);correlation between aging ?80 and three preoperative complication aggravation(p<0.05),namely hypertension,heart disease and respiratory tract infection,including obvious statistical significance with respiratory tract infection(p<0.01),while no statistical differences were observed with diabetes,urinary tract infection and hepatorenal dysfunction postoperative aggravation(p>0.05);correlation between aging ?80 and postoperative anemia,hypoalbuminemia,water and electrolyte disturbance and gastrointestinal reaction(p<0.05),including obvious correlation with anemia and hypoalbuminemia(p<0.01)and no statistical difference to uroschesis(p>0.05).Conclusion:1.Aging ?80,3d hospital stay before operation and surgery under general anesthesia were related to the complication occurrence among senior hip fracture patients with THA;2.Aging ?80 was related to the aggravaed preoperative complications of hypertension,heart disease and respiratory tract infection in hip fracture patients.Therefore,emphasis should be put on these kind of patients in clinical practice,especially the patients ?80 with respiratory tract infection;3.Hip fracture patients ?80 were prone to anemia,hypoalbuminemia,water and electrolyte disturbance and gastrointestinal reaction.Thus,after performing THA,symptomatic treatment need to be implemented for the patients,such as anemia correction,albumin supplement,regulation of water and electrolyte disturbance and specific treatment for gastrointestinal reaction.
