ObjectivesThe purpose of this study is to compare the changes of anxiety and self-care ability status of chronic renal failure patients before and after intervention with conventional nursing care when the seamless nursing was applied in arteriovenous fistula surgery of chronic renal failure patients,to explore the effect of the application of seamless nursing in arteriovenous fistula surgery of chronic renal failure patients and to provide references for the future nursing model of patients with chronic renal failure.MethodsIn this study,a randomized method was used to select 40 patients with chronic renal failure who were treated by arteriovenous fistula in two hospitals in Changchun and the clinical study was conducted in June to December,2016.These two hospital were divided into control group and l group by using the method of drawing lots.9 cases were lost during the study period,and finally 71 cases were included in the study,including 36 control cases and 35 treated cases.All patients were treated with forearm wrist radial artery-head vein fistula and were conducted with radial artery-head vein end anastomosis according to the standard by choosing arteriovenous end-to-side anastomosis.The anastomotic size was controlled at 8-12 mm.Patients in the control group only provided routine care.Intervention group of patients with routine care plus seamless care.The psychological status of the two groups was assessed before and after 4 weeks of intervention and 12 weeks after of intervention using the anxiety self-rating scale(SAS).The self-care ability of patients and the incidence of complications were assessed by self-care ability measurement scale(ESCA).To statistically analyze the data,SPSS19.0 was used and the data was analyzed by descriptive analysis.and the ?2 test was used to test the count data.The paired T test was used to compare the data of before and after the intervention,the two independent samples T test was used to compare the difference between groups.ResultsThere was no significant difference in SAS scores between the two groups before intervention.The SAS score of the intervention group was significantly lower than that of the control group at 4 and 12 weeks(P <0.05).The patients were followed up for 3months.Patients were followed up for 3 months to assess the patient's self-care ability measurement scale(ESCA).The results showed that the self-concept,self-care responsibility,self-care skills,health knowledge and self-care ability of people in the intervention group were significantly higher than those of the control group(P <0.05).Conclusions1.Seamless care can reduce patient anxiety levels.2.Seamless care can improve patient self-care ability.3.After the intervention of 4 weeks and 12 weeks,seamless care is better than conventional care in reducing the level of anxiety in patients.4.After the intervention of 12 weeks,seamless care is better than conventional care in improving the patient's self-concept,self-care responsibility,health knowledge level and self-care ability.5.During the observation period,the incidence of complications associated with arteriovenous fistula in the seamless care group was less than that in the routine care group.
