| ObjectiveTo analyze the incidence and risk factors of AKI(Acute Kidney Injury)in patients undergoing open heart surgery.Method4960 patients undergoing open heart surgery in the First Affiliated Hospital of Zhengzhou university during May.2014 to Dec.2016 were involved randomly in this study.2005 AKI network(AKIN)for the diagnosis and classification standard of AKI was used to group the patients: A group: 4790 patients with postoperative urine volume less than 0.5ml/(kg.H)for more than 6 hours.B group:170 patients without postoperative AKI.Further grouping was not completed.chi square test or Whitney U testwas used for comparison and logistic analysis was used for detection of independent risk factor.ResultThe total post-surgical AKI rate in patients undergoing open heart surgery was 3.4%.Univariate analysisfound the following relevantrisk factors of AKI:age,preoperative ejection fraction,preoperative coronary angiography within 7 days before surgery,diabetes,preoperative creatinine level,CPB use or not,intraoperative plasma transfusion,postoperative intubation time,postoperative blood pressure and postoperative balloon pump use.Logistic analysiswas used and confirmedthese independent risk factors:Preoperative creatinine level,preoperative artery 7 days after coronary angiography,left ventricular ejection fraction,postoperative hypotension,intraoperative and postoperative intubation time blocking time.ConclusionVarious factorsare associated with AKI in perioperative cardiac surgery.Comprehensive assessment of this factors and appropriate treatment are significant for the prevention of postoperative AKI. |
