Synovial membrane is an important component of the soft tissuein the capsule of temporomandibular joint.It plays an important role in maintaining the various functions of the temporomandibular joint.When the external stress is applied,the stability of the muscle chain in the temporomandibular joint is changed.It can cause the imbalance of the muscles around the joint,so that the synovial cells accept the external stimulation signal,directly or indirectly to produce some cytokines,causing the adaptation,remodeling and destruction of the synovium and other joint tissues.The IL-1beta and VEGF are common cytokines in synovial tissue,the expression of the supraphysiologic amount will be mediated by a series of chemical reactions,and induced immune response in target cells.The IL-1beta is one of the proinflammatory cytokines.It can directly bind to the receptors on the cell membrane,stimulate the production of various kinds of collagenases,and then cause local damage and aggravate the local inflammatory reaction.The IL-1beta can also indirectly participate in the mechanism of joint destruction by inducing the production of other cytokines.VEGF belongs to the family of platelet-derived growth factor,which can specifically act on the vascular endothelial cells,and promote the migration and proliferation of endothelial cells.The VEGF can also promote angiogenesis at the site of inflammation and maintain the inflammation of the joint,which causes the proliferation and congestion of the site of inflammation.It can induce and activate a variety of enzymes,change the permeability of blood vessels,and promote the release of the various inflammatory factors.Objective: The muscle chain of the oral and maxillofacial system is changed by the unilateral class II traction,which caused the changes of the temporomandibular joint and its surrounding tissues.By observating and analysing the histological changes of synovium,and the expression value of IL-1beta and VEGF in synovial tissue,explore the influence of joint' synovium of traction side in adult rats with unilateral class ?traction,so as to provide some references for clinical application.Method: 180 male 8-week-old SD rats were randomly divided into three groups:experimental group(group A),sham operation group(group B)and control group(group C),60 in each group.Group A was fixed Ni-Ti spring about 20 grams between the first molar and the upper incisor of right for establishing the unilateral traction model.Group B was fixed Ni-Ti spring about 0 grams in the function.The spring is not fixed in group C.Each group was randomly divided into 6 subgroups according to the time point(1d?3d?7d?14d?21d?28d),with 10 rats in each subgroup.Rats were sacrificed at the time points of each subgroup,then the right synovial tissue of double-plate area was taken for HE staining and immunohistochemical staining.Observe the histological changes of synovial tissue in each group.Detect the expression of IL-1? and VEGF among the groups.Results: The pathological score of group A compared with group B and group C was statistically significant(P<0.001).The IL-1? and VEGF of group A compared with group B and group C was statistically significant(P<0.001).There was a significant positive correlation(r1=0.912,r2=0.930,P<0.001)between the pathological scores and the expression of IL-1? and VEGF.Conclusion: Unilateral ? traction can cause histological changes of temporomandibular-joint' synovial tissue.There was a significant positive correlation between the histological changes and the expression of IL-1?,VEGF.
