In the complex liver surgery inevitably occur ischemia and reperfusion phenomenon,could cause serious liver damage,how to effectively reduce the liver injury caused by surgery is a clinically urgent problem to be solved.In recent years,medical gases have been widely used in clinical practice.Hydrogen?H2?has been shown to have not only antioxidant effects but also anti-inflammatory effects and play an important role in various organ diseases.At present,most of the researches on hydrogen-rich saline?HRS?are limited to small experimental animals such as rats and rabbits.There is little research on the close homotypic miniature pigs,and HRS is applied to the establishment of liver ischemia and reperfusion combined with liver part of the resection model has not been reported.Therefore,in this study,laparoscopic technique was used to establish the model of hepatic ischemia-reperfusion combined with partial hepatectomy,and HRS was used to monitor the changes of oxidative stress and inflammatory response after intervention andto explore the effect of HRS on hepatic ischemia-reperfusion combined with liver injury on the organism.In this study,24 miniature pigs were randomly divided into sham operation group?group A?,model group?group B?and HRS intervention group?group C?.Group A was only pneumoperitoneum and flipped liver lobe,group B established surgical model.The surgical model was established in group C and HRS?10 m L/kg?was injected into the portal vein at 10 min before perfusion,1 d,2 d and 3 d after operation.Blood samples were collected from preoperative,2 h after reperfusion,6 h after reperfusion,1 d,2 d,3 d,5 d and 7 d after operation.The blood samples were used to detect oxidative stress and inflammation.In this study,the successful operation of each group of surgery were not serious complications.Oxidative stress detection found: the levels of SOD,CAT,MPO and MDA in group A were significantly lower than those before operation?0.01<P<0.05?,then quickly recovered,indicating that anesthesia pneumoperitoneum on the organism's oxidative stress slightly and transient.The levels of SOD,CAT,GSH-Px and MPO in group B were significantly higher than those in preoperative andgroup A?P<0.01?.The levels of MDA in group B was higher than that of group A?0.01<P<0.05?.Indicating that the model on the organism in the early reperfusion can cause serious impact.The level of MPO in the C group was significantly higher than that in group B?P<0.01?,and the difference of MDA was significant?0.01<P<0.05?at 6 h after reperfusion and group B.Representation of HRS can reduce the oxidative stress caused by the model and reduce the lipid peroxidation product.Inflammatory index test found: the levels of WBC,LY,IL-6 and CRP in group A were significantly different from thosein preoperative at 6 h after reperfusion?P<0.01?.There were significant differences in the indexes of group B at 6 h after reperfusion and 1 d and 2 d after operation between preoperativeand group A?P<0.01?.LY was significantly different from that in group A at 1 d and 2 d after operation?0.01<P<0.05?.The results show that sham operation on the inflammation of the smaller and quickly restored after surgery,and the model caused by severe inflammation of the organism.The levels of TNF-?,IL-1?,IL-6,CRP and COR in group C were significantly higher than those in group B at 1 d and 2 d after operation?P<0.01?.The difference of IL-10 between the two groups was significantly higher than that of group B at 1 d,2 d,3 d and 5 d after operation?P<0.01?.The results showed that oxidative stress and inflammatory response in group C were lighter than those in group B.In summary,the findings indicated that HRS was applied to the model of hepatic ischemia-reperfusion with liver injury which laids the foundation for the clinical application of HRS and broadens the experimental animal model,and provids scientific theoretical basis for the study of the effect of HRS on hepatic ischemia-reperfusion combined with liver injury.HRS can effectively inhibit the viability of postoperative antioxidant enzymes,reduces oxidative stress caused by hepatic ischemia-reperfusion combined with liver injury.HRS reduces the postoperative inflammatory response by inhibiting proinflammatory factors,increases the expression of anti-inflammatory factors.
