Background:The cochlear implants(CI)were the main measure for the treatment of children with severe to extremely severe sensorineural hearing loss(SNHL).The children's auditory speech and social ability can be improved by postoperative rehabilitation.There were many factors that affect the rehabilitation effect of deaf children.At present,the influencing factors of rehabilitation effect were still not very clear after CI.Objective:To investigate the rehabilitation effect and influencing factors of severe to extremely severe SNHL children with CI.Method:A retrospective study of 113 severe to extremely severe SNHL children with CI from January 2013 to January 2015 in the First Hospital of Jilin University,Department of Otorhinolaryngology Head and Neck Surgery,and accepted rehabilitation treatment in Jilin Province deaf children rehabilitation center.The rehabilitation treatment effect was evaluated by categories of auditory performance(CAP),speechVI intelligibility rating(SIR)and auditory and verbal assessment criteria.To explore the relationship between implantation age,implantation ear,preoperative hearing aids,family rehabilitation time and the rehabilitation knowledge of parents and rehabilitation effect at 1 year.Result:After one year of rehabilitation treatment,(1)The CAP,SIR,language age and auditory ability of children were significantly improved compared with preoperative.(2)The development of CAP was 4(0-5)in2-4 years old group better than 3(0-5)in under 2 years group.The development of language age was 27.60(0-42)months in 2-4 years old group,which was higher than 21.60(0-42)in under 2 years old group.The language age delay was significantly difference between the three groups(p<0.05,p<0.05,p<0.05).(3)The choice of implant site had no effect on postoperative rehabilitation.(4)The 0-1h group compared with1-2h group and 2-3h group,there were significant differences in the CAP,SIR,language age and hearing development.But there was no significant difference between 1-2h group and 2-3h group.(5)Compared with different groups of rehabilitation knowledge,there was significant difference in the CAP and language age between the different groups,the unfamiliar group compared with familiar group and more familiar group,there was significant difference in the SIR and the development of hearing ability.Preoperative wearing hearing aids,the no wearing groupcompared with the single ear wearing group and binaural wear group,there was difference significant in the CAP,SIR and auditory ability development.And there was significant difference between the three groups in language age.Conclusion:(1)The rehabilitation training can improve the auditory and verbal abilities of deaf children after CI.(2)The suitable age for children with severe to extremely SNHL accept CI is 2-4 years old;(3)The choice of implantation ear has no obvious effect on the rehabilitation;(4)Family rehabilitation training has a positive effect,rehabilitation training knowledge should be aware,and the appropriate family rehabilitation training is necessary;(5)The deaf children should wear a single ear aids to accept intervention before CI.
