Objective:The study intends to compare the efficacy of dexmedetomidine and propofol for moderate sedation during spinal anesthesia in the lower abdominal surgery.Methods:There were 80 patients,American Society of Anesthesiologists grade?-?,scheduled for lower abdominal surgery under spinal anesthesia,were randomly divided into exmedetomidine group(D group)and propofol group(P group)according to different anesthetics,The initial10 min injection speed of D group was 1ug·kg-1·h-1and then maintained at a speed of 0.5ug·kg-1·h-1;The initial 10 min injection speed of P group was 6mg·kg-1·h-1,and then maintained at a speed of 2.5mg·kg-1·h-1.All patients were treated with standard program,and not used preoperative anesthetics.Patients who have any pain or discomfort should communicate with Anesthesiologist.The demographic information and recovery situation were recorded.The HR,MAP and OAA/S sedation scores of the two groups were recorded at the time infusing anesthetics instantly,and 5 min intervals until the end of surgery,The traction reaction of two groups,and the occurrence of adverse reactions were also observed.Results:1.There was no significant difference between the two groups in the general condition,but the effective analgesic time in D group(179.50 ±8.93 min)was significantly higher than that of group P(151.30 ± 8.29min)(P<0.01);The time of OAA/S score recovered to 4 points or above in D group(19.40 ± 2.48 min)was significantly longer than that in P group(13.93 ± 1.99 min)(P<0.01).2.There was no significant difference in OAA / S sedation score between group D and group P(P> 0.05).Both D group and P group were able to achieve moderate sedation after infusing anesthetics,but the time for group D to achieve moderate sedation was longer than group P,and the intraoperative sedationdepth of group D was higher than group P(P< 0.05).3.There was no significant difference in HR and MAP between group Dand group P(P> 0.05).HR in group Dafter anesthetics infusion was lower than group P(P<0.05),MAP in group Pwas lower than that of group D(P<0.05).4.The traction reactionofthe two groups wasslight,the depressed effect of traction reaction in group D was significantly better than the P group,the difference was statistically significant(P<0.05)5.There was no significant difference in the incidence of nausea,vomiting,bradycardia,hypotension and chills in the two groups(P>0.05).The incidence of respiratory depression in group Pwas significantly higher than that in group D(P <0.05).Conclusion:The sedative effect of dexmedetomidine in the lower abdomimal surgery under spinal anesthesia is better than propofol.
