Pneumonia is a common disease of pediatrics.Due to the anatomical and physiological characteristics of children,The respiratory system development is not mature,Extremely easy to progress to severe pneumonia.At present,Severe pneumonia is the important cause of death in infants and young children in our country.Cytokines Matrix metalloproteinases 9 and Matrix metalloproteinases inhibitor 1 are the main enzymes involved in the metabolism of extracellular Matrix.In recent years,The relationship between cytokines MMP-9 and TIMP-1 and the respiratory disease is becoming more and more attention.Study found,MMP-9 and its inhibitors TIMP-1 have played a certain role in inflammatory bowel disease of the respiratory system.The secretion and activity of TIMP-1 and MMP – 9 have changed in pulmonary inflammatory diseases.This study discusses the relationships among the cytokines of MMP-9 and TIMP-1 and severity of pneumonia,different etiology and recovery of moisture breathing lung function correlation by observing the infant pneumonia,peripheral blood serum levels of MMP-9 and TIMP-1 change,as well as the recovery of the moisture breathing lung function,And concluded that cytokines MMP-9 and TIMP-1 relationship with disease severity and prognosis,In order to provide objective basis to early clinical diagnosis,treatment and assessment after treatment.ObjectiveTo explore the expression and significance of Matrix metalloproteinases-9 and Tissue Inhibitors of Matrix Metalloproteinase-1 in serum at acute phase of severe pneumonia in infants.Methods45 infants of pneumonia were selected hospitalized in our hospital from October,2015 to May,2016.and divided into 20 cases of severe pneumonia group and 25 cases of mild pneumonia group according to the severity of the disease,and divided into virus group of 24 cases and the non-virus group of 21 cases on the basis of etiology;Additionally,20 cases of healthy children who accepting the physical examination at the same time were selected as the control group.Serum levels of MMP-9 and TIMP-1 were detected by using ELISA;Tidal breathing lung function was measured in all selected infant.Results1 Compared with control group,the serum levels of MMP-9 ?TIMP-1 and MMP-9/TIMP-1 ratio of the children with acute pneumonia were significantly higher,the severe pneumonia were higher than the mild pneumonia group,and the virus group were higher than the non-virus group(P<0.05);2.The ratio of the time to reach peak tidal expiratory flow to total expiratory time(TPTEF/TE)and the volume to reach peak tidal expiratory flow to total expiratory volume(VPTEF/VE)of pneumonia infants group in recovery period were significant lower than the control groups,severe pneumonia group were lower than that of mild pneumonia group,and the virus group were lower than that of non-virus group(all P<0.05);There were no statistically significant about VT/kg among three groups.3.The serum MMP-9 levels and the MMP-9/TIMP-1 molar are negatively correlated with TPTEF/TE and VPTEF/VE(r1=-0.459,-0.376;r2=-0.413,-0.327;all P<0.05).ConclusionsThe serum levels of MMP-9 and the MMP-9/TIMP-1 ratio in the acute phase of infantile pneumonia are correlated with the severity of pneumonia,the species of pathogens,and the damage degree of pulmonary function in recovery period,MMP-9 and the MMP-9/TIMP-1 ratio can be used as an objective indicator of prognosis.
