The Clinicopathological Characteristics And Risk Factors Of Thyroid Papillary Thyroid Carcinoma Complicated With Hashimoto’s Thyroiditis

Abstract/Summary:
Objective: To investigate the epidemiology,clinicopathological features and prognosis of patients with papillary thyroid carcinoma and thyroid papillary thyroiditis in patients with thyroid papillary thyroid carcinoma and thyroid papillary thyroiditis.Of clinical theory support.Methods: From January 2011 to December 2016,360 patients were diagnosed with thyroid papillary thyroidectomy and central lymphadenectomy at the Fifth People’s Hospital of Zhengzhou City(Zhengzhou People’s Hospital).According to the results of postoperative pathological examination with Hashimoto’s thyroiditis,the included cases were divided into thyroid papillary carcinoma with Hashimoto’s thyroiditis group 29 cases and simple thyroid papillary carcinoma group 331 cases.(Nodular size,glandular morphology,glandular border,lymph node,calcification,and lesion),thyroid function tests(FT3,FT4,TT3,TT3)were compared with those of PTC-HT group and PTC group.(AMES),prognosis(MACIS),clinical stage,and clinical stage(age,sex,pathological stage,lymph node metastasis,capsule invasion,extracapsular invasion,mass size,etc.),clinical stage and risk rating(AMES),prognosis(MACIS)The difference between.Results:(1)360 cases were included in the study,29 cases were PTC-HT group,and 331 cases were treated with PTC alone.There were 8 males and 21 females with an average age of 41.23 years.There were 142 males and 189 females,aged 25-69 years,with an average age of 43.4 years.Thyroid papillary carcinoma complicated with Hashimoto’s thyroiditis group(P <0.05).There was no significant difference between the two groups(P <0.05).There was no significant difference in the age of the papillary thyroid carcinoma group(P <0.05).There were 23 cases of thyroid enlargement in PTC-HT group,24 cases of thyroid masses,26 cases of neck pain,20 cases of neck compression,27 cases of dysphagia and 28 cases of hoarseness.There were 90 cases of thyroid enlargement,267 cases of thyroid masses,300 cases of neck pain,125 cases of neck compression,300 cases of dysphagia,329 cases of hoarseness.The incidence of thyroid enlargement and neck compression in PTC-HT group was significantly higher than that in PTC group(p <0.001).There was no statistically significant difference in thyroid masses,neck pain,dysphagia,and hoarseness(p> 0.05).(PTC-HT: 4.88 ± 2.06 pmol / L,PTC: 4.77 ± 3.59 pmol / L,t =-0.449),FT4(PTC-HT: 16.81 ± 2.69 pmol / L,PTC16.98 ±3.57 pmol / L,t =-0.449),TT3(PTC: 1.65 ± 0.36 nmol / L,PTC: 1.62 ± 0.37 nmol / L,t = 1.178),TT4(PTC-HT: 101.24 ± 21.01)Nmol / L,PTC: 99.87 ± 21.32 nmol / L,t = 0.827)were not statistically significant(p> 0.05).(PTC-HT: 93.29 ± 111.87,PTC: 34.12 ± 52.98),the difference was statistically significant(t =-5.458,P <0.001),and the expression level of TPOAb was significantly increased(PTC-HT: 4.06 ± 3.21,PTC: 2.81 ± 1.80), the difference was statistically significant(t =-2.489,P <0.05).(4)Compared with the PTC group(1.66 ± 1.19),the difference was statistically significant(t = 2.911,P <0.05)in the PTC-HT group(1.41 ± 0.69)compared with the simple thyroid ultrasonography There was no significant difference in gland morphology,gland border,lymph node,calcification and lesion status between the two groups(P> 0.05).(P <0.05).Conclusion: PTC-HT group(stage Ⅰ: 25 cases(86.2%),stage Ⅱ: 1 case(3.4%),stage Ⅲ: 2 cases(6.8%),1 case(3.4%(P <0.05).There was a significant difference between the two groups(P <0.05).There was a significant difference between the two groups(P <0.05).(T = 8.104,P <0.05).There was no significant difference in the number of lesions,lymph node metastasis,lesion size and tumor invasion between the two groups(P> 0.05).(2)The risk of PTC-HT(21.7%)was lower than that of PTC group(33.2%),and the difference was statistically significant(P <0.05).PTC-HT(mean 4.60 ± 0.77,score: 6.9%)was the same in the PTC group(mean score 4.88 ± 0.81,score ≥6 was 15.7%)in the MACIS system for risk stratification and prognosis assessment of PTC patients)Patients with low risk,the difference between the two groups was statistically significant(P <0.05).Conclusions: 1 In the case of Hashimoto’s thyroiditis,the invasiveness of thyroid papillary carcinoma did not increase.2 Tips women,Hashimoto’s thyroiditis may be the cause of thyroid papillary carcinoma.3 Combined with thyroid papillary carcinoma with Hashimoto’s thyroiditis in patients with a shorter prognosis than thyroid papillary carcinoma is better.
