Purpose:The treatment for Aortoiliac occlusive disease has reached to a new stage because of the appearance of endovascular technique.With the developing technique,including the advantage of high qualitive image technologies,precise equippments,manifold stents and baloons,the clinical results are stisfactory.Compared with the outcomes of traditional techniques,such as vascular bypass grafts technique,endovascular technique has its own advantages.Therefor,vascular surgeons begin to have the tendency of taking this technique scheme.Recently,a innumerous aortoiliac occlusive diseases have been treated by endovascular technique and the following up results are extremely satisfactory.As the renewal methods,vascular surgeons have also begun to make use it to treat complicated aortoiliac occlusive diseases by endovasuclar stenting or stenting combined with open sugeries.To further demonstrate the clinical outcomes of aortoiliac acculisives though endovascular techniques,according to TASC(Trans—Atlantic Inter—Society Consensus)guideline and revlevant literatures,we carry out the research about the curative effects of TASC C and TASC D aortoiliac occlusive diseases by the treatment of endovascular techinques.Methods and outcomes :Refering our department data from January 2012 to July 2016,we retrospectively study the clinical outcomes of aortoiliac acculisives though endovascular techniques or combine auxiliarily and necessarily with other techniques.According to TASC(Trans—Atlantic Inter—Society Consensus)guideline,our department have totally admitted and completed 367 cases,in which are244 cases classified as TASC type C and 123 cases as TASC type D.On the basis ofdifferent cases,32 cases were treated by balloon angioplasty and 200 cases by stenting:113 cases by self-expandable stents and 87 cases by covered stents,and 13 cases by subintimal angioplasty and 77 cases by endarterectomy combined with stenting and45 cases by hybrid procedures.In common iliac artery occlusive diseases,by angiography during operation,stents are put to the bifurcation of aorta with self-expandable stents or covered stents whose diameter are between.8mm to 10 mm.If in the cases of aortic occlusive disease,the diameter should be between 20 mm to 25 mm.All the processes of operation were smooth.Moreover,the rate of residual stenosis are below 30%,and the complication is lower than 2.4%.Postoperative Vascular doppler ultrasound and angiopraphy in necessary were carried out to follow up the patients.The outcome manifest that the primary patency rate in 12 th,18th 42 nd and 54 th months respectively are97.2/95.8?95.2/94.6 ?85.5/84.3?79.3/77.8?77.6/75.5(TASC C&D).And its secondary patency rate are 100%.Conclusion:Endovascular technique is a safe and effective methods for treating aortoiliac occulive diseases of TASC C and TASC D;and it can be a primary choice.
