Objective:To investigate the correlation between lipid accumulation index and islet ?cell function in middle-aged and elderly people.Materials and methods:According to the selection and exclusion criteria,10109 participants were selected from the Dalian community residents and our sample was derived from the Chinese Patients with Type 2 Diabetes Cancer Risk Epidemic Studies which was held in Dalian central Hospital by Chinese Medical Association.And the data were collected from August 2011 to December 2011.All the community residents were 40 or more years old.Height,weight,waist circumference(WC)were measured.The levels of triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density protein cholesterol(HDL-C),fasting blood glucose(FBG),2 hours postprandial blood glucose(2hPG),Fasting insulin and glycosylated hemoglobin(HbAlc)were tested.The lipid accumulation index(LAP),HOMA-IR,HOMA-? were calculated.According to the LAP quartile divided into Q1,Q2,Q3 and Q4 group,compare the prevalence of diabetes between groups.The relationship between LAP and HOMA-IR and HOMA-? was described by curve fitting model.Finally,the effects of LAP on HOMA-IR and HOMA-? were proved by correlation and linear regression.Results:1.The prevalence of diabetes in the population was 23.3%,The prevalence of diabetes mellitus in each groups was 11.4%,18.5%,25.6%and 37.5%,the difference was statistically significant.The risk of diabetes in Q4 group was 4.564 times higher than that in Q1(OR = 4.654,95%CI:4.018-5.390,P<0.05).2.The LAP and HOMA-IR curve fitting showed that the relationship between the LAP and HOMA-IR showed a linear correlation,With the increase of LAP,HOMA-IR gradually increased.The curve equation is:Y = 0.16549X + 68.4089.3.Spearman correlation analysis between LAP and HOMA-IR showed that HOMA-IR was positively correlated with LAP.Multivariate linear regression analysis was performed with HOMA-IR as the dependent variable and LAP as the independent variable,After adjustment for age,sex,LDL-C and HDL-C,LAP could enter the equation,the difference was statistically significant(P<0.05),The equation is HOMA-IR = 2.066 + 0.014LAP +0.012 age-1.040HDL-C + 0.183LDL-C.4.The curves of LAP and HOMA-P were fitted.The results showed that,The relationship between LAP and HOMA-? showed an inverted U trend.With the increase of LAP,HOMA-? increased first,then entered the platform and then decreased slowly.The curve equation is:Y =-0.000829578X2 + 0.37585X + 61.47068.5.The LAPs were classified according to the quartile,and each category was analyzed as a separate data.Spearman correlation showed that:In group Q1,HOMA-?was positively correlated with LAP,The difference of HOMA-P and LAP between Q2 and Q3 group was not statistically significant,In group Q4,HOMA-P was negatively correlated with LAP,The difference was statistically significant(P<0.05).Multivariate linear regression analysis was performed with HAPA-? as the dependent variable and LAP as the independent variable.The results showed that:After adjustment for age and sex,LDL-C and HDL-C,the LAP in Q1 and Q4 could still enter the equation,the difference was statistically significant(P<0.05),The regression equation was:The regressive equation for group 1 was HOMA-P = 86.345 + 0.688LAP + 14.378 Sex-0595 age-8.486HDL-C,and the regression equation for group Q2 was HOMA-?=134.52-0.067LAP + 17.58 Sex-0.546 age-15.384HDL-C.Conclusion:1.with the increase in LAP,diabetes prevalence rate gradually increased2.with the increase in LAP,insulin resistance gradually increased.3.With the increase of LAP,pancreatic ?-cell secretion activity compensatory increase to decompensation and then to the process of weakening secretion.
