| Purpose:This article explores that the relationship between different syndrom types of TCM diabetic foot patients and blood fat,uric acid,cystatin C indexes of patients.It offeres modern objective indicator for Chinese tradition medicine type of diabetic foot patients.Material and method:It collected 80 diabetic foot patients from medical records,divided into five syndromes:deficiency of qi and blood,the dampness-heat toxic pattern,hot poison injury yin,deficiency of liver-yin and kidney-yin,spleen-kidney yang defeciency.It collected nondiabetic 22 patients(excluding those with diabetes,hypertension and gangrene)as the control group.It registers basic information about the patients admission number,name,xex,course of disease,diagnosis,the first biochemical examination indicators including blood glucose,blood fat,uric acid,cystatin C.Statistics with SPSS21.0 software,count data using X2 test,measurement data using t test,multiple sets of mean comparison using ANOVA analysis and LSD test,and carries on the preliminary analysis basis of the correlation.Results:1.In the collection of 80 cases,average age of the patients is 70.391±12.33 years of age;the course of diabetic above 5 years is about 81.25%,and above 10 years is about 47.50%;about 81.25%diabetic foot patients in treatment when it becomes aggravation(above 2 grade);95%diabetic foot patients'blood glucose is high;the dampness heat toxic type with the greatest number,account about 27.5%,and the following are deficiency of liver-yin and kidney-yin(25%),deficiency of qi and blood(22.5%),hot poison injury yin(16.25),spleen-kidney yang defeciency(8.75%).2.The group of nondiabetic is higher than group of diabetic foot in indicators of total cholesterol(TC),high-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C),the comparing is statistically significant(P<0.05),The type of deficiency of qi and blood get higher triglycerides(TG)and low-density lipoprotein(LDL-C),compared with group of the dampness-heat toxic pattern and spleen-kidney yang defeciency have statisticallly significance(P<0.05).And the total cholesterol(TC),low-density lipoprotein(LDL-C),triglyceride(TG)of spleen-kidney yang defeciency is low,with qi and blood deficiency was statistically difference(P<0.05).And triglyceride(TG)of deficiency of liver-yin and kidney-yin is highest,with the dampness-heat toxic pattern and pleen-kidney yang defeciency are statistically difference(P<0.05).In terms of blood fat the hot poison injury yin of low high-density lipoprotein(HDL-C),compared with deficiency of qi and blood was statistically significant(P<0.05)3.the diabetic foot group of high uric acid,compared with non-diabetic group was statistically significant(P<0.05),deficiency of liver-yin and kidney-yin,spleen-kidney yang defeciency type of uric acid is higher than that of hot poison injury yin,was statistically significant(P<0.05).4.The diabetic foot group of high cystatin c,compared with non-diabetic group was statistically significant(P<0.05),that the greater Wagner level was accompanied with the higher Cys C;spleen-kidney yang defeciency of cystatin c is high,more than the other three groups was statistically significant(P<0.05).Conclusion:1.The incidence of diabetic foot is telate to course of diabetes disease and blood glucose.Diabetes foot education is very important,should allow patients to pay attention to diabetic foot disease,careful care of the foot,early detection,early treatment,can reduce the difficulty of treatment and expenditure,reduce the amputation rate.2.High levels of uric acid is ralation to the occurrence of diabetes mellitus;diabetes mellitus type of hot poison injury yin syndrome is characterized by low uric acid,deficiency of liver-yin and kidney-yin syndrome is characterized by high uric acid.3.In this study,diabetic patients with good blood lipid control,but non-diabetic elderly lack of lipid management,poor blood lipid control.deficiency of qi and blood syndrome group with total cholesterol,triglyceride,low density lipoprotein cholesterol is characterized by diabetes foot,deficiency of qi and blood syndrome blood lipid metabolism disorder the most obvious.4.Serum CysC was positively correlated with the severity of diabetic foot with ulcer.Diabetic foot syndrome in spleen-kidney yang defeciency get the highest Cystatin C,deficiency of liver-yin and kidney-yin get the lowest cystatin C. |
