Objective: Patients with periodontal prosthesis are commonly found periodontal tissue destruction,which easily causes red,swallow and inflamed gums or severe gingival recession,leads to unstable occlusion and pathological tooth displacement.This kind of unbalanced occlusal force leads to bite collapse,moreover,the alveolar bone destruction leads to poor crown-root ratio,which causes secondary occlusal trauma.Therefore,decreasing the probability of secondary occlusal trauma is the first priority of periodontal prosthesis treatment.For those poor distributed residual teeth,using removable overdenture provides cross arch design and better force distribution of the roots.This kind of treatment can reduce the resorption rate of alveolar bone and enhance the stability and support of denture.With the progress of the implant design and the popularization of the clinical application,the successful rates of periodontal prosthesis treatments are outstandingly increased.The aim of the study was to investigate the effect of the occlusal reconstruction by using different implant-assisted repair method in patients with tooth loss caused by periodontal disease by observing and analyzing clinical cases.Methods: Ten case reports were carried out in this study.The time of study collection of information was from February 2012 to October 2015.Patients had mild to severe periodontitis and respectively used implant supported overdenture,removable overdenture or magnetic attachment supported overdenture.Among them,there were 5 cases of implant supported,4 cases of removable and 1 case of magnetic attachment supported.In addition to the implant surgeries,the application of surgical methods such as crown lengthening,periodontal flap operation,root resection,and hemisection were used combined with the treatments.Patients were being followed up after 6 months of treatment.The evaluation of each treatment method was made according to the cost,esthetic appearance,chewing function and convenience of the overdenture.Results: These 10 cases were reconstructed in the way of an implant-assisted prosthesis.Although some patients considered the economic and physical factors and chose localized dental implants.By using the customized treatment plans,we were still able to use the way of a small number of implant-assisted prosthesis to improve the fixation of reconstruction.All cases were satisfied with the treatment results after more than half a year follow up and the implants also perfectly achieved the purpose of repair.Conclusion In this study,Dr.Amsterdam's idea of an aggressive periodontitis treatment with restoration and prosthesis therapies was represented in the form of implant-assisted prosthesis in recent years.It greatly increased the treatment options in the treatment plan of periodontal patients with tooth loss.The different treatment plans of the implant-assisted prosthesis,showing different effects in the periodontitis patients with tooth loss during occlusal reconstruction,had its pros and cons.Implant supported overdenture has the best results,the highest satisfaction of patients,but the price is expensive,prolong treatment time.Removable overdenture is easy to take on and take off and has lower cost;however,it is the second of the chewing effect and acceptable esthetic.Although the repair time due to implant surgeries requires a longer recovery period,in the overall treatment results,whether it is simple,comfortable and durable,have a obvious and positive effect.
