Object:Intracranial aneurysms are the first cause of subarachnoid hemorrhage,and posterior communicating aneurysms are more prone to rupture and bleeding.Previous studies have explored the prognosis of patients underwent aneurysm surgery,and lacks the relevant prognostic study of interventional therapy.At the same time,the literature often involves intracranial multi-site aneurysm research,and lacks more specific relevance.The overall analysis of the aneurysm occurs the impact of confounding factors,resulting in unavoidable bias.This study was conducted on the posterior communicating aneurysm,based on the patient's comprehensive and accurate clinical data.The morphological data of the aneurysm were analyzed comprehensively to explore the relationship between the different influencing factors and the prognosis of the patients.We can provide a theoretical basis for more comprehensive and accurate assessment of treatment to develop optimal treatment regimen and improve the therapeutic effect and improve the prognosis of patients by this study.Method:Select the patients in our department from January 2014 to January 2017 with the established inclusion criteria of posterior communicating aneurysm.1.Exclude the intracranial aneurysms caused by trauma and infection;2.Exclude other cerebrovascular diseases,such as arteriovenous malformations,smoke disease;3.Bring the internal carotid artery-posterior communicating artery cystic aneurysm into our study.All patients were followed up for 3 to 12 months,and patients were asked the re-admission after 6 to 12 months to research the postoperative changes of posterior communicating artery aneurysms by DSA.All patients were followed up to research the prognosis of posterior communicating aneurysms.A total of 72 patients were included in the case group,including 23 male patients and 49 female patients.All of them were confirmed by postoperative CTA or DSA as posterior communicating aneurysms.The data of the clinical inpatient records and follow-up information of the patients are brought into the case group analyzed retrospectively.The GOS scores were the dependent variables and what could affect the prognosis of the patients: age,sex,history of hypertension,diameter of the aneurysm,Fisher CT classification,Hunt-Hess scale,postoperative complications are used as independent variables,then rank,test and construct the Logistic regression model,screening for the prognostic significance of the prognosis.Result:The patients were hospitalized for 13 days on average.The patients were discharged with GOS score to evaluate the prognosis of the patients.Among them,43 patients were cured,17 were self-care patients,11 were severely disabled,1 were vegetative being,0 were dead,accounting for 59.7%,23.6%,15.3%,15.3%,0respectively.There was no significant difference in the two gender groups(P> 0.05),the aneurysm tumor diameter(P> 0.05),the different directions of dome(P> 0.05).But there are significant differences in age(P <0.01),history of hypertension(P <0.01),history of subarachnoid hemorrhage(P <0.01),Fisher CT grade(P <0.01),postoperative complications(P <0.01)and Hunt-Hess scale(P <0.01).The difference between the six indexes and the prognosis of the patients was statistically significant,and then we bring them into the multiple Logistic regression analysis to screen the variables.The results show that the prognosis of patients are influenced deeply by Hunt-Hess grade(P <0.01)and postoperative complications(P <0.01).Conclusion:Hunt-Hess scale and postoperative complications are the risk factors for the prognosis of patients with posterior communicating aneurysms after interventional therapy.It can be used to evaluate the prognosis of patients with posterior communicating aneurysms after interventional therapy and help in guiding the optimal clinical treatment.
