Ob j ect i ve:Sleep apnea hypopnea index?AHI?fromThe polysomnography?PSG?as the evaluation index to measure sleep apnea syndrome of this disease has been widely recognized by the international performance,but also within the scope of identification,monitoring leads numerous and data interpretation is vast,the sleep monitoring results of the interpretation,identification is crucial to the monitoring results,the accuracy of artificial recognition is far better than machine.The problem:there are many artifacts that need professional and technical personnel to identify every frame per to 30 seconds.Interpretation of standards available.Patients feel difficulty to normal sleep because of the leads and the sleep environment changed.the leads fall of when body moved during sleep due to loose data collection and so on.we need time and energy to get the index which can evaluate the disease,but the fluctuation range of the index is large and difficult to guarantee the accurate evaluation of the disease,for there are so many uncontrollable factors.This study mean to consider the relationship between Hs-CRP and PSG monitoring indicators by serological detection of Hs-CRP in patients with sleep apnea,in order to find the objective relatively stability monitoring and biochemical index for evaluating the severity of sleep apnea,to search for a parallel to the international standard index to assist assessment of disease severity,to conduct a comprehensive assessment of sleep apnea syndrome.Methods:From October 2014 to January 2017,the First Affiliated Hospital of Dalian Medical University,207 patients with suspected sleep apnea in the outpatient department of Otolaryngology did the overnight sleep monitoring.The collected data of body mass index?BMI?and oxygen desaturation index?ODI4?,time spent below oxygen saturation of 90%?TS90%?and serum Hs-CRP level do the correlation analysis with the international standard of sleep apnea index AHI;If using the AHI>5 times/h as a standard in the diagnosis of OSAS,determine the most suitable ODI4 with high sensitivity and specificity.;further determined the the corresponding ODI4 with AHI which can represent different disease severity so we can get risk factors of evaluating sleep apnea severity through multi factor regression.Result:1.The analysis of Pearson and Spearman showed the correlation between sleep apnea index?AHI?had positive correlation with BMI,abdominal circumference,neck circumference,ODI4,TS90%and Hs-CRP?r=0.403,r=0.397,r=0.41,r=0.891,r=0.721,r=0.272,P<0.05?.2.Compared to the traditional index AHI,Evaluated OSAS with ODI4?5 times/h,the sensitivity and specificity were 86.7%and 92.6%.The corresponding values of AHI15 and 30 of ODI4 were 10 and 19,respectively.3.The analysis of Pearson and Spearman showed the correlation between sleep apnea patients serum Hs-CRP were positively correlated with AHI,neck circumference,ODI4,TS90%,BMI and abdominal circumference.?r=0.272,r=0.248,r=0.312,r=0.254,r=0.298,r=0.25,P<0.05?.4.Further multivariate stepwise regression analysis of variable screening results showed that BMI?T,=2.434,P=0.016?,and ODI4?T,=2.592,P=0.01?,correlated with Hs-CRP,and were independent risk factors.Conclusions:1.ODI4 itself contains oxygen index as the sleep apnea index?AHI?can accurately reflect the severity of sleep apnea.2.ODI4 value of 5 is equivalent to AHI value of 5,ODI value of 10 equivalent to AHI value of 15,ODI4value of 19 equivalent to AHI value of 30.3.Serum level of Hs-CRP may be affected by BMI?P=0.033?and ODI4?P=0.049?,is consistent with the pathogenesis of OSAS.Chronic inflammatory reaction Systemic of multiple organ system induced by obesity and intermittent hypoxia.4.The ODI4 and the level of Hs-CRP can be used as an index to evaluate the severity of sleep apnea.The results of these two indexes are different from each other,not subject to interference.
