Objective: To observe the difference of the positive rate of hepatitis B surface antibody among different age groups.Looking for HLA polymomorphism with the B surface antibody correlation.And to find out factors affecting the leve of hepatitis B surface antibody by using the comparison between total antibody activity and antibody affinity of the patients with positive hepatitis B surface antibody in the three different age groups,and the relationship between total antibody activity and antibody affinity and serum biochemical indexes.Methods: the serum samples were from the patients(hepatitis B surface antibody positive,hepatitis B surface antigen negative,hepatitis B core antibody negative,hepatitis B e antibody negative,hepatitis B e antigen negative)in the first affiliated hospital of dalian medical university and the dalian children's hospital,and a total of 200 serum samples were selected.In which,males and females accounted for 100 cases respectively.The samples were divided into four groups according to their ages: 1-2 years old group,males and females accounted for 25 cases respectively;6-8 years old group,males and females accounted for 25 cases respectively;24-26 years old group,males and females accounted for 25 cases respectively.In hepatitis B surface antibody negative control group of 6-8 years old group,males and females accounted for 25 cases respectively.At first test HLA-DQB1-genotyping in the group of 6-8 years old.By using the LIASON full-automatic chemical luminescence analyzer,hepatitis B surface antibody positive serum were detected after serial dilutions according to a certain proportion,until the detecting results of diluted samples were negative,then the total activity and affinity were obtained through a linear transformation.The serum biochemical items of the four groups were detected by using Hitachi 7180 full-automatic biochemical analyzer.The correlation between the total activity of antibody and the affinity of antibody and the biochemical indexes was found out.At the same time detecting the 200 alleles of 6-8years group in HLA-DQB1.The other selected 70 cases of serum samples test hepatitis B surface antibody and hepatitis B surface antigen Neutralization experiment.In which,adult group(hepatitis B surface antibody positive,hepatitis B surface antigen negative,hepatitis B core antibody positive,hepatitis B e antibody positive,hepatitis B e antigen negative)are 35 samples,chirlden groups(hepatitis B surface antibody positive,hepatitis B surface antigen negative,hepatitis B core antibody negative,hepatitis B e antibody negative,hepatitis B e antigen negative)are 35 samples.Results: HLA DQB1-polymorphism and the level of hepatitis B surface antibody have no statistical relationship.The total activity of hepatitis B surface antibody in 1-2years old group was correlated with iron protein,aspartate aminotransferase,serum total protein,Serum albumin,serum uric acid,serum urea nitrogen,serum calcium,serum magnesium and cystatin C.the total activity of hepatitis B surface antibody in 1-2 years old group was not correlated with serum alanine amino transferase ammonia,blood glucose,creatinine,phosphorus,NT – pro BNP.The antibody affinity of 1-2 years old group was related to serum alanine aminotransferase and aspartate aminotransferase.The antibody affinity of 1-2 years old group was unrelated to total protein,albumin,blood sugar,urea nitrogen,creatinine,uric acid,calcium,magnesium,phosphorus,urinary inhibition C,ferritin NT – pro BNP.The total activity of 6-8 years old group was correlated with serum urea nitrogen,serum cystatin C.The total activity of 6-8 years old group was not correlated with alanine amino acid,aspartic acid,total protein,albumin,uric acid,blood glucose,creatinine,calcium,magnesium,phosphorus,iron,protein,NT– pro BNP.The affinity of 6-8 years old group was correlated with serum alanine aminotransferase,serum total protein,blood glucose,serum urea nitrogen,serum creatinine and serum phosphorus.The affinity of 6-8 years old group was not correlated with alanine amino acid,aspartic acid,albumin,uric acid,calcium,magnesium,urinary inhibition C,ferritin,NT-pro BNPThe total activity of 24-26 years old group was correlated with serum albumin and serum calcium.Serum uric acid,phosphorus and white blood cells were the factors affecting the antibody production in hepatitis B surface antibody positive group and negative group of 6-8 years old group.There are significant differences from recovery adult and healthy children in hepatitis B surface antibody's activity.Conclusion: The neutralization of the leve surface antibody induce by vaccine was lower than naturally infected persons,and the positive rate and total activity of hepatits B surface antibody,nutrition and development of people at different ages.
