Purpose: To observe the changes of brain electrical activity in cortex and subcortex during the disappearance of consciousness induced by propofol and analyze the influence of propofol in different brain regions by microelectrodes implanted in the brain of patients with epilepsy. In order to provide some evidence to ascertain the central location of propofol.Method: Twenty-six patients with epilepsy who had undergone microelectrodes implantation with ROSA were selected. The patients were implanted with frontal lobe,hippocampus,insula and temporal lobe. Because of the limitation of operation,the number and location of electrode implantation were divided into 4 groups according to the location of electrode implantation. There were 12 cases in group A (electrode implanted in frontal lobe), 12 cases in group B (electrode implanted in hippocampus),10 cases in group C (electrode implanted in insula), 12 cases. in group D (electrode implanted in temporal lobe). Among these patients,there was 1 case planted in the frontal lobe,hippocampus,the insula and the temporal lobe at the same time ;lease planted in the frontal lobe,cingulate cortex and the amygdala ;lcase planted in the cingulate cortex and thalamus nucleus Afer the patients entering the operating room,and the nurses established the intravenous access. Monitor the heart rate (HR),systolic blood pressure (SBP), diastolic blood pressure (DBP), pulse oximetry (SP02),bispectral index (BIS), high-lead EEG were connected with the electrode of the patient and begin to record the stereotactic EEG(SEEG) for 2 minutes, then give propofol 4-5?g / ml with target-controlled infusion . When the consciousness of the patient disappeared and the BIS value dropped to 60, continued to record the SEEG signal for 2 minutes. Finally, Comparied the different changes of brain electrical spectrum. between cortex and subcortex.Result:, The spectrum showed that the energy of ?,?,?,? wave in each brain area was increased and the difference was statistically significant compared with the awake state. The change of energy in each brain area was statistically different,the frontal lobe energy changes the highest, while the temporal lobe energy changes the lowest. As can be seen from the electrical spectrum chart: after anesthesia,the energy of frontal and cingulate was higher than the amygdala, the energy of cingulate was higher than the thalamus.Conclusion: In the process of unconsciousness caused by propofol, , The energy of the frontal lobe, hippocampus, insula and temporal lobe increased highly while the frontal lobe is the most higher and the temporal lobe was the least. After anesthesia,the brain electrical signal of thel cortex was more sensitive than the subcortex.
