ObjectiveThis study aims to make a System evaluation for traditional internal f ixation fusion surgery and Transforaminal Lumbar Interbody Fusion(TLI F)about the treatment of senile lumbar degenerative disease.MethodsRetrieve the Chinese biomedical literature database(CBM:January 2015-December 2016),WANFANG DATA(January 2015-December 2016),MEDLINE(January 2015-December 2016),China hownet(CNKI:January 2015-December 2016),PUBMED(January 2015-December 2016),the Netherlands medicine(Embase:January 2015-December 2016)such as database,Search about Transforaminal Lumbar Interbody Fusion and the Traditional internal fixation fusion surgery comparison literature research for the treatment of lumbar degenerative disease.By two researchers read carefully and select all relevant prospective randomised controlled trials and retrospective study,extract data using RevMan5.3.After analysis,the conclusion after analyzing the data,discussed the result comes in.ResultsCarefully read and screening,10 prospective studies and 1 article retrospective study.The results:1.The operation time of contrast:the MIS-TLIF group and OPEN surgical treatment of lumbar degenerative disease operation time of the comparison results show that two kinds of surgical operation time difference have no statistical significance(P =0.98),prompted the surgery time of the two groups has no relative merits.2.The intraoperative blood loss,postoperative lead flow and the comparison of incision length:Two groups of surgery of the difference in the intraoperative blood loss,postoperative lead flow and the length of the incision was statistically significant(P<0.00001,P = 0.0002,P<0.00001),the analysis results show that the MIS-TLIF surgery about intraoperative blood loss,postoperative lead flow and the length of the incision than traditional surgery has obvious advantages.3.The hospitalization time of the treatment of lumbar degenerative disease of minimally invasive TLIF group significantly lower than the traditional open group(P<0.00001).4.Compare the occurrence probability of complications,compared two groups has no statistical significance(P =0.36).5.Postoperative march,minimally invasive group VAS score and ODI scores are lower than traditional open group,most can show that minimally invasive group of patients recover faster than traditional open group.6.MIS-TLIF group and OPEN group at follow-up of postoperative time(more than one year)with lumbar pain VAS score and dysfunction index ODI score comparison:the comparison has no statistical significance(VAS:P =0.10,ODI,P=0.09).ConclusionsEvaluation results through this system,this study suggests that compared with traditional internal fixation fusion surgery,intraoperative blood loss,minimally invasive TLIF operation time,postoperative flow,length of hospital,incision lengthstay and postoperative VAS and ODI scores were significantly lower,and in terms of probability of occurrence of complications and the forward curative effect between the two has the sa,the long-term clinical effect is basically the same.Minimally invasive TLIF surgery procedures development time is not long,had a wide range of promotion and popularization,the surgeon is proficient in the operation of the master degree is not a benchmark,and outcome indicators have different standard,lead to the end of the literature index of heterogeneity is large,so the system research institute made the conclusion should be treated with caution,looking forward to later can have a more rigorous research method,higher quality,and on the basis of large sample randomized controlled studies of MIS-TLIF and traditional open surgery on the contrast of fusion and internal fixation surgery to make more persuasive.
