BackgroundGastrointestinal dysfunction is a common disease in the digestive system disease,the disease is not a set of independent disease,but a part of multiple organ dysfunction(MODS).Gastrointestinal dysfunction is common in surgical diseases.Its common causes are infection or noninfectious diseases as well as iatrogenic factors(such as major surgery,anesthesia complications or continual total parenteral nutrition,etc).With the actualization of the two-child policy in our country,cesarean sections,especially the repeated cesarean sections because of the scar uterus increased.For patients after abdominal surgery,postoperative complications of abdominal or pelvic adhesion is a common one.Abdominal or pelvic adhesion is often found in repeated cesarean sections clinically.Such surgeries are more difficult,take more time and more surgery trauma.Therefore,it' s becoming more and more common to be complicated by gastrointestinal dysfunction in such delivery women after the operation.However,the recovery of gastrointestinal function,as the starting part of maternal whole rehabilitation after cesarean section,play an important role in promoting maternal eat early,improve airframe nutrition and immune system,reduce the perioperative bad mood,and promote the secretion of milk and uterine instauration,incision healing.It has important clinical significance in improving the quality of the delivery women' s life after the operation.For this kind of women,more active intervention measures should be taken in order to reduce postoperative gastrointestinal dysfunction complications.At present there is no such corresponding reports and research in China,and the active intervention of TCM treatment on the disease has obvious advantages,and should be further studied.ObjectiveThis research follows the DME principle and method and adopts randomized controlled method trials,combined with the improvement of the American society for reproductive medicine(ASRM)adhesion classification standard,on the basis of western medicine routine treatment after cesarean section,by comparing the patients postoperative bowel sound recovery time,anus exhaust time,defecation time and TCM syndrome scale score to evaluate the clinical curative effect of ordinary acupuncture for the recovery of gastrointestinal function after repeated cesarean section in scar uterus and complicating with abdominal or pelvic adhesion.And then to provide the basis for the further clinical application and promotion.MethodsThrough randomized controlled method trials,80 inpatients of selective repeated cesarean section complicated with abdominal or pelvic adhesion from January 2016 to December 2016,in Obstetrics,Nanhai Maternity and Children's Hospital affiliated to Guangzhou University of Chinese Medicine were selected and randomly divided into control group and treatment group.At the end,76 cases were incorporated into the statistics,39 cases of the control group and the other of the treatment group.Both groups were given treatment and care according to the clinical pathway of elective cesarean delivery.Based on that,treatment groups were given with the acupuncture at Zusanli(both),Shangjuxu(both),Gongsun(both),Zhongwan(both),Tianshu(both),etc 6 hours after operation by the same physician,interval of 6 hours,twice a day until the flatus.After treatment,the two groups were observed the time of postoperative bowel sounds,flatus,bowel movement,abdominal distension grade,gastrointestinal reaction grade and the clinical efficacy.All data would be analyzed by the SPSS22.0.Result1.Begin the trial,two groups' basic date(such as maternal age,defecate habit,pregnancy body mass index,number of cesarean delivery,estational age,preoperative white blood cell count and hemoglobin)were compared.The results showed that all of them had no statistical significance(P>0.05).The two groups are comparable.2.After the trial,The treatment group' s average time of postoperative bowel sounds,flatus,bowel movement were significantly ahead of that of the control group(P<0.05).The treatment group' s abdominal distension grade,gastrointestinal reaction grade were significantly better than that of the control group(P<0.05).It had marked effect in treatment group(P<0.05).ConclusionAcupuncture used to promote the cesarean mothers' recovery of gastrointestinal function after repeated cesarean section complicated with abdominal or pelvic adhesion can get better results.Through promoting the recovery of gastrointestinal function can promote maternal eat early,improve airframe nutrition and immune system,reduce the perioperative bad mood,and promote the secretion of milk and uterine instauration,incision healing,then to improve the quality of the delivery women' s life after the operation.So we can draw a safe conclusion that the common acupuncture used actively as intervented treatment for such women is an effective,safe and convenient method and worth promoting.
