The Clinical Application Analysis Of Flexible Bronchoscopy In Children With Chronic Cough

ObjectiveTo explore the clinic value and safety of flexible bronchoscopy in children with chronic cough.MethodsThe clinical data of 75 case of chronic cough with flexible bronchoscopy from July 2015 to September 2016 in the pediatric department of our hospital were retrospectively analyzed.ResultsIn 75 patients with chronic cough,20 case(26.7%)were caused by cough variant asthma(CVA),18 case(24.0%)by airway dysplasia,10 cases(13.3%)by protracted bacterial bronchitis(PBB),9 cases(12.0%)by upper airway cough syndrome(UACS),8 cases(10.7%)by airway foreign body,4 cases(5.3%)by recurrent respiratory tract infections,3 cases(4.0%)by CVA combined airway dysplasia,2 cases(2.7%)by vascular ring and 1 case(1.3%)of idiopathic pulmonary hemosiderosis(IPH).In 21 cases of airway dysplasia,10 cases of tracheobronchomalacia(TBM)were found,5 cases of airway opening anomaly,3 cases of bronchial stenosis and 3 cases of TBM combined airway opening anomaly.Bacterial culture of bronchoalveolar lavage fluid(BALF)were performed in 42 times and 5 samples(11.9%)were positive,2 samples were streptococcus pneumoniae,2 samples were haemophilus influenza,1 sample were pseudomonas aeruginosa.Cytology examination of BALF showed the percentage of neutrophil cells was significantly higher in the PBB group than in the CVA group and the other diseases group(P<0.05),the percentage of eosinophil cells was significantly higher in the CVA group than in the PBB group and the other diseases group(P<0.05).8 cases had their airway foreign bodies removed through flexible bronchoscopy and cough obviously alleviated.The complcations rate of flexible bronchoscopy was 17.3%(13/75).ConclusionFlexible bronchoscopy has an important role in the etiological diagnosis and therapy of chronic cough in children,and it was a safe and effective method,especially to diagnosis for airway dysplasia,airway foreign body and PBB.
