ObjectiveTo investigate the differential diagnosis of granulomatous mastitis and breast cancer by mammography and magnetic resonance imaging(dynamic enhancement technology,diffusion weighted imaging).MethodsCollected from January 2013 to December 2016,Guangdong Provincial Hospital of Chinese medicine to accept breast mammography or magnetic resonance examination of granulomatous mastitis and breast cancer female patients,age range 26-82 years,mean age 42.3 years,a total of 139 cases.Which received a total of 116 patients with breast mammography(pathology diagnosed as granulomatous mastitis in 54 cases,62 cases of breast cancer),received a total of 48 nuclear magnetic resonance examination of the breast(pathology diagnosed as granulomatous mastitis in 22 cases,breast cancer 26 cases),the molybdenum target images and magnetic resonance images of all cases were obtained,and the imaging morphological features,intensification methods,signal changes,TIC curves and ADC values were analyzed for granulomatous mastitis and breast cancer.The TIC curves were used to classify the granulomatous mastitis and breast cancer,and the sensitivity of the TIC curve to the two diseases was calculated.The TIC curve was used to analyze the sensitivity of the TIC curve to the granulomatous mastitis and breast cancer.Specificity,coincidence rate,positive predictive value and negative predictive value.The sensitivity,specificity,diagnostic coincidence rate,positive predictive value and negative predictive value of molybdenum target radiography and magnetic resonance imaging in the diagnosis of granulomatous mastitis and breast cancer were calculated and compared.All the statistical data were analyzed by SPSS22.0 software Analysis,the use of chi-square test or t test to P<0.05 to determine the difference was statistically significant.Result1.The sensitivity of granulomatous mastitis was 75.8%,the specificity was 69.1%,the positive predictive value was 73.8%and the negative predictive value was 71.3%.The chi-square test,P<0.05,can be considered the morphological manifestations of lesions can be used as a differential diagnosis of granulomatous mastitis and breast cancer standards.2.The sensitivity of the TIC curve was 61.5%,the specificity was 77.8%,the positive predictive value was 92.3%and the negative predictive value was 31.8%.The chi-square test was used to detect the granulomatous mastitis and breast cancer,P<0.05.3.The diagnostic threshold was 90.9%,88.5%and 89.6%respectively.The t test method was used to evaluate the best cutoff value of the two lesions with ADC=1.017×10-3mm2//s,P<0.05.4.TIC curve and ADC value of the two lesions were combined diagnosis,the sensitivity of 96.5%,specificity of 68.8%.ConclusionFrom the morphological analysis,granulomatous mastitis and breast cancer have their own unique morphological features,but there are similar overlap;breast mammography X-ray on the small calcification of high recognition,can be used as early diagnosis of breast cancer The time-signal intensity curve,diffusion-weighted imaging and ADC value of magnetic resonance dynamic enhancement have certain diagnostic value for granulomatous mastitis and breast cancer.
