Risk Factors Analysis Of Postoperative Rucurrence Of Stones In Patients Wth Choledocholithiasis By ERCP Treatment

Objective:To investigate the risk factors of recurrence after choledocholithiasis ERCP lithotomy,and to provide effective measures for clinical treatment and preventionMethod:A total of 226 choledocholithiasis patients with ERCP lithotomy at Yanbian University hospital(2013.03-2016.06)were recruited for clinical informationcollection.Cases with inclusion criteria were divided into recurrence group and non-recurrence group.Single factor statistical analysis was conducted among relevant factors,such as gender,age,dietary habit,body mass index,biliary tract sur gery history,stone diameter/number,duodenal diverticula,biliary duct stenosis and common biliay duct dilation degree.Then the observed parameters from single factor analysis(P<0.05)was substituted into Logistic multi-factor regression analysis.Statistical values of P and 95%CI were calculated in order to identify the independent risk factors of recurrence.Result:ln the 226 collected cases,except 2 cases of death and 13 cases of losing contact,a total of 211 cases met the inclusion criteria.There were 26 cases of recurrence after ERCP resulting in a recurrence rate of 12.32%.By single factor analysis,statistical significances in biliary tract surgery history(P=0.001),stone numbers(P=0.031),duodenal diverticula(P=0.001),common biliary duct diameter(P=0.005)were observed between recurrence group and non-recurrence group(P<0.05).Logistic regression analysis identified three factors with statistical significance:1.Biliary tract surgery history(OR:3.211,95%CI:1.329-7.760,P=0.010);2.Common biliary duct diameter?15mm(OR:3.268,95%CI:1.179-9.062,P=0.023);3.Combining with duodenal diverticula(OR:4.336,95%CI:1.525-12.329,P=0.006)Conclusion:Risk factors of recurrence among choledocholithiasis patients with ERCP were previous history of biliary tract surgery,combining with duodenal diverticula and common biliary duct diameter?15mm.
