The Analysis Of Influencing Factors And TCM Syndrome Types Of Hydrocephalus After ASAH

Objective1.To study the risk factors of hydrocephalus after aSAH.2.To analysis of correlation between hydrocephalus and TCM syndromes after aSAH?MethodsUsing the method of retrospective case study.According to the inclusion and exclusion criteria of this study,the patients who was diagnosis with aneurysmal subarachnoid hemorrhage at encephalopathy,encephalopathy three,encephalopathy five and Fangcun department of Neurology in Guangdong Province Traditional Chinese Medical Hospital from May 2006 to May 2016 were included,collecting the date of normal information(age,gender),medical history data(smoking history,drinking history,history of hypertension,whether bleeding into the ventricles,aneurysm location,admission CT-Fisher grading,admission Hunt-Hess grading),Yin or Yang syndrome and admission TCM syndrome,hydrocephalus after aSAH is use as outcome indicators,than single factor analysis and multi factor unconditional stepwise logistic regression analysis are use to study the effects of the above indexes on outcome indicators,meanwhile,analyzing the correlation between TCM syndrome and hydrocephalus after aSAH.Results1.After screening a total of 291 cases were included,of which the incidence of hydrocephalus in 48 cases,accounting for 16.5%of the ratio,did not occur in patients with hydrocephalus,accounting for the total number of 83.5%cases.2.Univariate analysing of age,gender,smoking history,history of drinking,history of hypertension,hemorrhage into ventricle,aneurysm location,admission CT-Fisher score,admission Hunt-Hess grading,acute hydrocephalus,and get the results of the age,the hemorrhage into the ventricles,the location of the aneurysm,the admission of Fisher grade,and the admission of Hunt-Hess can affect the formation of hydrocephalus after aSAH;multivariate analysis show that age,hemorrhage into the ventricles,aneurysm location,admission Hunt-Hess are the independent risk factor for hydrocephalus after aSAH.3.On the Traditional Chinese Medicine,a total of 276 cases of Yang Syndrome,which including Hyperactivity of liver Yang,wind heat syndrome in 249 cases,Fire disturbing orifices syndrome 19 cases,the lest of a total of 8 cases of syndrome in 291 patients;Yin Syndrome in 15 cases.Yin Syndrome accounted for 5.2%of the total,Yang Syndrome accounted for 94.8%.48 cases of hydrocephalus patients,Yang type syndrome in 15 cases,Yin type syndrome in 6 cases;in the absence of hydrocephalus in patients with the disease,Yang Syndrome in 234 cases,Yin Syndrome in 9 cases.Single factor analysising show that Yin and Yang Syndrome,Liver-yang hyperactivity,Wind&Fire disturbance were statistically significant,and multi factor analysis of hyperactivity of liver Yang,wind and fire disturbance are independent risk factors?Conclusion1.The incidence of hydrocephalus is 16.5%,which is cause by many complicated factors.The age,hemorrhage into the ventricles,aneurysm location,admission Hunt-Hess classification are independent risk factors.The incidence of aSAH was mostly in women,but gender isn' t a risk factor for hydrocephalus.The patients of aSAH with the above independent risk factors,we should pay more attention to regular review,early detection,early treatment,to improve the patients condition.2.The patients of aSAH accounted for the vast majority of yang Syndrome,the proportion of up to 94.8%.Yang Syndrome are contain of hyperactivity of liver Yang,fire disturbance Syndrome and Fire disturbing orifices mainly.It is reflect by wind and fire on early stage,which is may relate to excess of liver Yang be internalize into the wind,blood stasis stagnation and heat-transmission,Wind and fire encourage each other,finally,blood overflow pulse.Therefore,early treatment with dispelling wind,clearing heat and fire.Hyperactivity of liver Yang,wind and heat disturbance as an independent risk factor for hydrocephalus after aSAH,we still pay attention to this type of patients with hydrocephalus,early detection and early treatment.
