Objective: To explore the risk factors of hypotension after tracheal intubation in patients with severe acute respiratory distress syndrome,and to provide theoretical basis for the treatment of the disease.Methods: Selected since February 2014 to 2016 years 2 months in our hospital underwent tracheal intubation treatment of 110 cases of severe acute respiratory distress syndrome patients were studied,according to whether the occurrence of hypotension hypotension group and normal blood pressure group.Of which 34 patients with hypotension group,systolic blood pressure decreased to 90 mm Hg or decline more than 40 mm Hg;76 cases of normal blood pressure group,systolic blood pressure in mm Hg above 90 mm Hg)and the decline rate is not more than 40 mm Hg(including 40 mm Hg).Observation were compared between the two groups of patients with general information,before and after tracheal intubation basic vital signs and The risk factors were identified by single factor analysis and multivariate logistic regression analysis was used to determine the correlation between risk factors and blood pressure drop.Results: Weight of two groups of patients,body mass index,before intubation systolic blood pressure,before intubation diastolic pressure and intubation after diastolic pressure difference is significant,with statistical significance(P < 0.05).In addition,hypotension group before intubation in the patients of the two groups of patients of the APACHE II score no significant difference(P > 0.05),after intubation(P < 0.05),with statistical significance.The patients in the two groups before intubation plasma albumin differ significantly(P < 0.05),with statistical significance(P < 0.05).Multi factor regression analysis showed that,weight,body mass index,before intubation systolic blood pressure,before intubation Shu Zhang pressure and blood Plasma albumin is closely related to the low blood pressure after intubation,and it has statistical significance(P<0.05).Conclusion: Low body weight,low body mass index,low systolic pressure before intubation,low diastolic pressure and low albumin level were the risk factors of hypotension after tracheal intubation in ARDS patients.Therefore,emergency physicians should pay attention to risk factors before intubation,and take preventive measures to avoid the occurrence of hypotension after intubation,improve the success rate of treatment of patients.
