Clinical Analysis Of Curative Effect Of Febuxostat On Gouty Arthritis And Its Safety Study

Objective:To observe the efficacy of febuxostat in the treatment of gouty arthritis and thecharacteristics of ultrasound image of the affected joints before and after treatment,investigate the improvement of affected joints of gouty arthritis with febuxostat,and observe the joint changes of gouty arthritis.Methods:100 male patients with gouty arthritis were selected as the subjects according to selection criteria,and divided into febuxostat group(febuxostat 40mg/d,n=55)and allopurinol group(allopurinol 100mg/d,n=55)according to the situation of drugs.The serum uric acid(UA),lipid levels of the subjects in two groups and the changes of synovial membrane surrounding blood flows signals and ultrasound images of the affected joints before and after treatment were compared.The occurrence of attack gout and adverse reactions were recorded.Results:Compared with before treatment,the UA levels of patients in two groups were both decreased after treatment,there was significant difference(P<0.05);At16,20,24 weeks,the percentage of the patients whose serum UA level reached the target value in febuxostat group was higher than that ofin allopurinol group(P<0.05).Compared with before treatment,thelevel of high density lipoprotein(HDL)in febuxostat group after treatmentwas increased(P=0.001);There were no significant diffferences in the levels of triglycerides(TG),total cholesterol(TC)and low density lipoprotein(LDL)between before and after treament in febuxostat group(P>0.05).There were nosignificant difference in TC,TG,LDL and HDL in allopurinol group between before and after treatment(P>0.05).Compared with before treatment,the treatment of the articular effusion,UA crystal and synovial thickening were improved in the two groups,there was significant difference(P <0.05).The articular effusion at acute stage of gout,synovial membrane surrounding blood flows signals and synovial thickening of the the patients was improved,but there was no significant difference between the two groups(P>0.05).Most of the UA crystal disappeared after treatment of the patients in two groups(febuxostat group 54.54%,allopurinol group 35.56%).Febuxostat was better than allopurinol in dissolving UA crystal with acute gout attack,and there was significant difference between the two groups(P<0.05).There were no significant changes at chronic gout attack.There were gout attack in two groups during the treatment,the number of gout attacks decreased gradually with the prolongation of time,but there was no significant difference between the two groups(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Febuxostat and allopurinol could both reduce UA levels,but the effect of febuxostat on reducing UA levels is better,could also improve the performance of acute gout,and effectively dissolve the crystal.Febuxostat could improve dyslipidemia,increase the level of high density lipoprotein(HDL),Gout attack rate and adverse reactions are similar in both febuxosat and allopurinol.
