Object:To investigate the clinical significance of the diagnosis of Parkinson's disease by transcranial sonography;to study the correlation between the echo of substantia nigra and the age,onsetage and course of disease in patients with Parkinson's disease;to clarify the difference of substantia nigra echo between Parkinson's disease with different initial symptoms.Patients and methods:A total of 78 patients with Parkinson's disease,who were admitted to No.1 Hospital of Jilin University from December 2015 to~2016 in September,were enrolled in the group of patients with disease,12 of them were not involved in the statistical evaluation because of the unilateral or bilateral temporal window penetration.All of the patients were diagnosed by the expert in the study of Parkinson's disease in the Department of Neurology,according to the diagnostic criteria of Parkinson's disease in China.A total of 21 healthy volunteers were selected in the same period,into the healthy control group,3 of them were not involved in the statistical evaluation of the temporal windowpenetration.There was no significant difference in age and sex between the Parkinson's disease group and the healthy control group.The transcranial sonography examination of the above subjectswere examined by the experienced doctor Liu,Department of Neurological Head and neck vascular ultrasound Center.The examiners were unaware of the clinical condition of the subjects.Data of two groups were collected,Including gender,age,age and course of disease in patients with Parkinson's disease,midbrain area measured by transcranial sonography,substantia nigra echo intensity,substantia nigra area and the width of the three ventricle.Results:There was a difference in the ability of temporal window penetration between genders and ages(P < 0.05);significant difference were detected in the echo area and substantia nigra echo intensity between the Parkinson disease group and the healthy control group(P < 0.05);in Parkinson's disease group and healthy control group,there was no difference in the midbrain area and the width of the third ventricle;the echo area of substantia nigra was not correlated with age,age of onset and course of disease;the echo intensity of substantia nigra was correlated with the echo area of substantia nigra(P < 0.05);there was no difference in echo intensity and echo area of substantia nigra between the sexes;between the different symptoms of Parkinson's disease,there wasno difference in the echo area.According to the different symptoms of Parkinson's disease,the patients were divided into two groups,patients with tremor as the first symptom were included in tremor group,Patients with non tremor onset symptoms,including tremor,rigidity,postural instability as the first symptom,were included in the non tremor group,and the differences between the two groups were compared.Conclusions:1.SN echo intensity and SN+ area were significantly different between the PD group and the healthy control group,TCS could be used as an effective screening method for screening and diagnosing PD;2.TCS is of limited diagnostic value for the elderly,especially for elderly women,who tend to have poor temporal window penetration.
