Objective:Stroke is a major cause of adult-onset disability and dependency.We investigated whether quantitative electroencephalography(QEEG)are of prognostic value for functional outcome 6 months after ischemic stroke.And we try to identify the QEEG index and threshold value which can most accurately discriminate between non-survivors and survivors.Methods:Twenty-two patients presenting with acute severe supratentorial ischemic stroke were incrementally included within the first 72 hour of clinical evolution.We performed QEEG monitoring and NIHSS with24h-72 h.Follow-ups were done after 6 months and the patients' outcomes were assessed by the modified Rankin scale(m RS)scores.Clinical characteristics,volume of ischemia and EEG parameters were correlated with functional outcome assessed with the modified Rankin Scale(m RS)score.Predictive values for moderate disability,severe disability and death were calculated using receiver operating characteristic(ROC)curves.1.We found that relative delta power,delta/alpha ratio,(delta+theta)/(alpha +beta)ratio,NIHSS and area of infarction were positively correlated with the m RS scores at 6 months.And relative theta power,relative alpha power negatively correlated with the 6-month scores.Results:2.There was a significant difference between patients who died and patients who survived in area of infarction,NIHSS,relative delta power,relative alpha power,relative theta power,delta/alpha ratio and(delta+theta)/(alpha +beta)ratio.In ROC analysis,death was predicted by relative delta power with overall accuracy of 86.36%.3.There was a significant difference between those with mild disability and severe disability.The moderate disability group was predicted by relative theta power with an overall accuracy of 92.86%.Conclusions:EEG may be of prognostic value for the functional outcome in the acute setting of acute severe supratentorial infarction.
