Object With the continuous improvement of living standards,life style transformation and social advancement of the aging process in China,The prevalence of elderly patients with type 2 Diabetes is a rapidly rising trend.It has become the most important chronic disease which endangers the safety of people's life after tumour and vascular disease.Type 2 diabetes mellitus and its various acute and chronic complications can cause high morbidity and mortality,It has become the third largest threat to the health of the world's people,seriously affecting the quality of the patients' life.Transient ischemic attack(TIA)is one of the most important independent risk factors for ischemic stroke,the frequent occurrence of TIA is considered to be the super alarm signal of cerebral infarction in recent years,it has been proved that about 4% to 8% of cerebral infarction occurs after transient cerebral ischemia.Uric acid(UA)is the final product of purine,which is now known to be the direct cause of gout,But studies have found that high levels of uric acid in the body not only can lead to gout,Uric acid can also affect glucose and lipid metabolism,atherosclerosis,inflammation in the body,change the blood coagulation status mechanism and effect of transient ischemic attack patients eventually develop into cerebral infarction in patients with type 2 diabetes mellitus.The relationship between serum uric acid level and type 2 diabetes mellitus patients with transient ischemic attack(TIA)in elderly patients with 93 type 2 diabetes mellitus was studied,to explore the role of uric acid in the pathogenesis of cerebral infarction,for the prevention and treatment of cerebral infarction to carry out new directions.Method A retrospective analysis was performed on 93 elderly patients with type 2 diabetes mellitus complicated with transient ischemic attack in the First Affiliated Hospital of Zhengzhou University from September 2014 to ~2016 in April.Methods 30 healthy persons in our hospital were selected as control group.At the same time,30 patients with diabetes mellitus without TIA were selected as diabetic group.All the data were collected,including height,weight,age,and body mass fraction;At the same time,fasting blood glucose,glycosylated hemoglobin,total cholesterol,triglyceride,low density lipoprotein,high density lipoprotein and serum uric acid were measured and compared.Results Comparison of basic data and biochemical indexes in control group,the non TIA group and the TIA group:There was no significant difference in age and sex between the control group,non TIA group and the TIA group.The biochemical indexes in the three groups showed that the fasting blood glucose,BMI,Hb A1 c,TC,TG,LDL-C in the non TIA group and the TIA group were significantly higher than those in the control group,and HDL-C was lower than that in the control group,the difference was statistically significant(P<0.05).The fasting blood glucose,Hb A1 c,TC,LDL-C,uric acid in the TIA group were higher,HDL-C was lower than those in the non TIA group.the difference was statistically significant(P<0.05).The fasting blood glucose,Hb A1 c,TG,LDL-C and UA in group C were higher than those in group A and group B,HDL-C was lower than that in group A and group B,LDL-C and uric acid in group B was higher than that in group A,HDL-C was lower than that in group A,the difference was statistically significant(P<0.05).There was no statistical difference between the three groups of BMI and TC. Person correlation analysis showed that serum uric acid level in the TIA group was positively correlated with FPG,BMI,TG and LDL-C(P < 0.05),and negatively correlated with HDL-C(P < 0.05).Logistic regression analysis of type 2 diabetes mellitus complicated with TIA showed the non TIA group and the TIA group as a whole,whether to merge TIA as dependent variable,age,BMI,FPG,Hb A1 c,TG,TC,HDL-C,LDL-C,uric acid as independent variables,logistic regression analysis,the results showed that there was a statistically significant uric acid,HDL-C(P < 0.05),suggesting that uric acid is a risk factor in patients with type 2 diabetes with TIA,HDL-C is a protective factor.Multiple linear regression analysis of type 2 diabetes mellitus complicated with TIA showed that that fasting plasma glucose(?=13.240,P<0.05)?TG(? =9.822,P<0.05)?BMI(?=4.478,P<0.05)is the influencing factors.Conclusions The high level of blood uric acid in aged patients with type 2 diabetic patients with recurrent TIA and development are the risk factors of cerebral infarction,HDL-C is a protective factor,SUA can be used as one of the assessment of elderly type 2 diabetes and the risk of TIA index,so as to provide the basis for clinical work.
