Background At present,the majority of patients with smoking should cease from smoking regularly before operation.But researches of the relationship between nicotine dependence and postoperative pain are still less.Long term nicotine exposure can change the structure and function of the pain pathway,and may cause nicotine tolerance,which results in opioid tolerance in patients with nicotine dependence,and increases opioids consumption.How to provide nicotine dependent patients better postoperative pain has important significance for reducing postoperative,depressing the consumption of opioids,abating respiratory complications,improving lung function,enhancing comfort,promoting postoperative recovery.Therefore,the purpose of this study is to investigate the effect of postoperative analgesia on postoperative pain and postoperative opioids consumption in smoking patients undergoing radical operation for thyroid cancer.Method A total of 80 cases of thyroid cancer patients from June 2016 to November 2016 in our hospital were selected.They were divided into two groups according to FTND score before surgery: low nicotine dependence group(group L)and high nicotine dependence group(group H).And then we divide them into two groups randomly according to postoperative analgesia: group superficial cervical plexus(group C)and group PCIA(group I).The consumption of sufentanil,pain VAS score and Ramsay score at 1 minutes after extubation(T1),before leaving PACU(T2),24 hours after the surgery(T3)and 48 hours after the surgery(T4)were recorded.We also observe PONV,complications after cervical plexus block,postoperative pulmonary complications and LOS and so on.Results The pain VAS score of group HI is significantly higher than that of group LI at cough of T1 and T2 and T3(P?0.05).So are group HC and group LC.The consumption of remifentanil during the operation in group HI and group LI are separately significantly higher than that in group HC and group LC(P?0.05).The consumption of sufentanil after the surgery in group HI is significantly higher than that of group LI at T2,T3 and T4(P?0.05).So are group HC and group LC.And the consumption of sufentanil of group HI and group LI are respectively higher than group HC and group LC(P?0.05).Conclusion The patients receiving radical resection of thyroid cancer having higher nicotine dependence,they will undergo higher postoperative pain and require more postoperative consumption of analgesic drugs.General anesthesia combined with superficial cervical plexus block can effectively reduce postoperative consumption of analgesic drugs in smoking patients.
