ObjectiveTo summarize the diagnosis and treatment of 45 children with congenital esophageal atresia(CEA)in our hospital,explore the related factors of postoperative complications in children with congenital esophageal atresia,provide guidance for the clinical treatment and complications of the disease,improve the survival rate and quality of life of children with esophageal atresia.Materials and methodsThe clinical data of children with congenital esophageal atresia were diagnosed from September 2010 to September 2016 in the First Affiliated Hospital of Zhengzhou University.47 cases of congenital esophageal atresia were recruited.Of these patients,27 cases were boys and 20 cases were girls.Due to the esophageal detachment distance greater than 3cm and the merger of high anus,parents of 2 cases refused to surgery,gave up the treatment.According to clinical classification,19 cases were type IIIa CEA,28 cases were type IIIb CEA.The prognosis was evaluated by outpatient review and telephone follow-up.The statistics included gestational age,birth weight,concomitant malformations,Clinical classification,surgical methods,enteral nutrition,postoperative complications.The data were analyzed by SPSS 19.0 software,and analysis the relationship between gestational age,birth weight,concomitant malformation,clinical classification,surgical methods,early enteral nutrition and postoperative complications,survival rate.Results45 cases received one-stage anastomosis surgery.14 cases of complications after surgery,31 cases did not appear complications.Comparison of full-term infants and premature infants,the standard weight of children and low birth weight children,open surgery and thoracic surgery in children,children with early enteral nutrition and total parenteral nutrition and complications between the two groups there was no statistically significant difference(P>0.05).Compared with pneumonia and uninfected pneumonia in children,the blind end distance is greater than 2cm children and the blind end distance less than 2cm children,concomitant malformations and without malformations in children,the differences between the two groups were statistically significant(P<0.05).There was no significant difference between the two groups in the operation time and the surgical day age in the patients with complications and no complications(P>0.05).The time and expenditure of hospitalization between the early enteral nutrition group and the total parenteral nutrition group were compared,the difference was statistically significant(P<0.05).Conclusion1.Pneumonia and associated malformations,especially severe cardiovascular malformations,can significantly increase the incidence of postoperative complications in children with congenital esophageal atresia.2.Blind distance is closely related to the postoperative complications of children.Preoperative evaluation should be accurate and appropriate surgical time should be selected.3.The use of early enteral nutrition support is of positive significance in shortening the length of stay and reducing the medical costs of children with esophageal atresia.
