Avascular necrosis of the femoral head is a common orthopedic disease,often reduce the quality of life of patients,and bring financial burden to the family.It's common causes:trauma,alcohol,hormones,and so on.The incidence of the disease showed an upward trend[1].In recent years,due to the widespread use of hormones,the incidence of steroid-induced femoral head necrosis is highly,and many experimental studies have shown that hormonal causes of femoral head necrosis in the femoral head of bone marrow stromal cells(BMSCs)decreased,but also Confirmed that the hormone will affect the activity of bone marrow stromal cells,So that the differentiation ability of bone marrow stromal cells decreased.Therefore,the study of bone marrow stromal cells with steroid-induced femoral head necrosis is deepening,and even consider that hormone-induced femoral head necrosis is associated with bone marrow stromal cells related to a class of diseases.ObjectiveTo observe the effect of autologous platelet-rich plasma-induced bone marrow stromal cells combined with the same osteochondral scaffolds on the treatment of early sterile femoral head necrosis in rabbits and to explore a new method for the treatment of early femoral head necrosis.Materials and MethodsUsing the anticoagulant-containing vacuum blood collection tube from the experimental rabbit central artery to take arterial blood 5 ml,the platelet-rich plasma(PRP)was obtained by centrifugation.The bone marrow was extracted from the autologous femoral cavity of the rabbits by the 16 th bone marrow aspiration needle.Using 5 ml of bone marrow density gradient centrifugation,you can get rabbit bone marrow stromal cells,and primary culture and osteogenesis induction of cells.Then,using experimental rabbits that their platelet-rich plasma(PRP)and bone marrow stromal stem cells(BMSCs)were prepraed to make femoral head necrosis model with hormones.Taking the same kind of femoral condyle bone cartilage to produce cartilage scaffold.Finally,bone marrow stromal cells(BMSCs)induced by osteogenesis were transplanted into cartilage scaffolds to form cell-scaffold complexes.The bone-coredecompression was performed on the necrotic femoral head with the 16 th bone marrow puncture,and the cell-scaffold complex was implanted into the necrotic femoral head.Animals were grouped together,with a total of 4 groups and different groups taking different interventions.Group A was blank control;group B was only decompression of core decompression;group C was decompressed into osteoblast-induced cells after decompression;group D was decompressed and implanted with cell-scaffold complex,each group of 20 rabbits.After the test at weeks 2,4 and 8 weeks,each group were killed 6,6,8 rabbits.Experimental rabbits' s femoral head were examinated in general specimen and Histological.ResultsThe treatment effect of D group was the most significant,and the difference between group D and other groups was significant(p <0.05)ConclusionBone marrow stromal cells differentiate into osteoblasts induced by platelet-rich plasma.Cartilage scaffolds provide a good spatial structure for differentiation of bone marrow stromal cells into osteoblasts in order to differentiate into osteoblasts.Bone marrow stromal cells induced by platelet-rich plasma and cartilage scaffolds Application can promote the repair of early rabbit steroid-induced femoral head necrosis.
