Clinical Study Of "Gumibao" Curing Necrosis Of Femoral Head In Early-middle Metaphase And Experimental Study On Its Pastille Serum Effects On External Osteoclasts

Purpose:1.Randomized study by collecting clinical cases,observation of Yishen Huoxue Gumibao security prescription in the treatment of early stage osteonecrosis of the femoral head(Osteonecrosis of the femoral head,ONFH)theclinical curative effect.2.Through the experimental study,detection of serum containing secret shares toxicity;get enough quantity,stable characters meet the test requirements of the osteoclast induced bone marrow mononuclear cells in vitro by way of observation;security medicated serum on the femoral bone marrow mononuclear cells differentiate into osteoclast;observing the effects of serum on bone stock security cells of bone resorption.Methods:1.In accordance with the scope of Western inclusion criteria and exclusion criteria,from January 2016 to February 2017 in the city of Suzhou orthopedics hospital of traditional Chinese medicine outpatient treatment and follow-up of ONFH patients,43 cases(51 hips),were randomly divided into two groups of patients,the control group oral alendronate(Fosamax)70mgqw8 weeks the treatment group oral;Bulgaria shares dose,4 weeks for a course of treatment,a total of 2 courses.Before and after treatment,the Harris score,pain visual analogue scal e(VAS)and i magi ng exami nati on resul ts were recorded i n detail.2.Bone marrow mononuclear cells in vitro induced by alpha,-MEN solution containing 25ng/ml M-CSF and 100ng/ml of bone marrow mononuclear cells in cultured and passaged in RANKL,1%PS and 5%FBS were used to induce osteoclast;during induction by the inverted microscope for morphology observation,while using tartrate resistant acid phosphatase(TRAP)for identification of osteoclasts staining;CCK-8 method was used to assay the serum containing toxic secret shares;bone marrow mononuclear cells induced by paraformaldehyde fixed by TRAP staining to calculate the number of positive cells;bone slices were stained by toluidine blue to observe the bone formation and resorption of the number and areea of scanning observation;finally,electron microscopy of bone lacuna.Results:1.the Harris score and VAS score before and after treatment in the two groups were significantly improved compared with bedfore treatment(p<0.05),but there was no significant difference between the two groups.2.the two groups before and after treatment the necrosis index score difference studies showed that in the relief process of osteonecrosis of the femoral head,the control group of alendronate treatment group curative effect is better than western medicine secret shares.3.the two groups before and after treatment results in relieving bone marrow edema and j oi nt dffusion,have certa n curati ve efect but shares d ose protecti on,and the control group of Western medi d ne al endronate has no obvi ous advantage compared to.4.through bone marrow mononuclear cells in vitro induced by the way,can meet the experimental requirements of osteoclasts.5.secret serum groups compared with the saline control group,12h,24h,3D and 5D four in the observation period there were no significant differences(P>0.05).6.through the observation of TRAP positive cells in the inhibition of bone marrow mononuclear cells differentiate into osteoclast function,the concentration of serum group shares security compared with the saline control group had significant difference(P<0.05),but no significant difference in efficacy between the femoral security groups(P>0.05).7.in the limit of bone resorption counting function,through data analysis,the high concentration stock secret serum group and saline control group curative effect was significantly different between(P<0.05);the rest of the curative effect of each group showed no significant difference.8.in the restricted area of bone resorption,through data analysis,the high concentration stock secret serum group and saline control group curative effect was significantly different between(P<0.05);there were significant differences between group serum concentration and low concentration and high concentration of serum containing groups(P<0.05);between other groups effect no significant difference.Conclusion:1.for earl y stage osteonecrosi s of the femoral head of pati ents,have certain curati ve effect of femoral density Bulgaria can alleviate pain,improve hip function,reduce bone marrow edema and joint effusion,femoral head necrosis delayed collapse process,the effect is similar to alendronate.2.secret drug serum can inhibit the bone marrow mononulear cells differentiate into osteoclasts;can inhibit osteoclastic bone resorption.
