Analysis On The Structure Variation And The Influencing Factors Of Tuberculosis Patients' Hospitalization Expenses In A Specialist Hospital For Ten Years

Objective: This paper discusses the composition of hospitalization expenses of tuberculosis patients in a specialist hospital from 2006 to 2015,and discusses the trend of hospitalization expenses of tuberculosis patients in a decade.This paper analyzes the basic situation,the payment method and the disease factors of tuberculosis patients in a specialist hospital from 2006 to 2015,and discusses the influencing factors of hospitalization expenses.In order to explore the measures to reduce the tuberculosis medical expenses,improve the tuberculosis control policy and measures to provide a scientific reference to reduce the burden of patients with disease.Methods: The research object selected the hospital inpatient of tuberculosis who hospitalized from 2006 to 2015 and the hospitalization cost was studied.The study included demographic characteristics of tuberculosis patients,payment methods of medical insurance,disease factors and hospital costs and the composition of the various costs.Descriptive statistics is using the arithmetic mean,median,standard deviation and interquartile range to analyze;composition of hospital costs and the costs of using the composition ratio will be described;hospitalization costs and fees with the use of changes in the time structure change,structural change value and structural change contribution rate were analyzed statistically.The influencing factors of hospitalization cost of tuberculosis patients were analyzed.Non-parametric rank sum test was used.Spearman correlation test was used to analyze the multivariate analysis.Multivariate analysis was performed by multiple linear regression model.All statistical analysisusing SPSS19.0 software package.Results: 1.Statistical analysis of demographic characteristics showed that male patients?71.3%?which were more likely than women?28.7%?in the basic situation of tuberculosis hospitalization who aged between 2 and 115 years,with an average age of47.67 ± 18.70 years old,the hospital area of the majority of patients is accounting for74.53% of the total number of patients and the average length of 17d?median?.There were 12,954 cases?43.9%?of the basic medical insurance for urban workers.There were 98.2% of the patients who had been admitted to hospital.The number of patients without surgery was 96.7% of the total number of patients.2.The average number of inpatients in a specialist hospital was 2954,and the number of hospitalized patients was on the rise and with an average annual growth rate of 7.3%;the total tuberculosis patients hospitalization costs is 26033.5504 million in2006-2015,per capita hospital costs is 7930.38 yuan.Total hospitalization costs and per capita hospitalization costs are on the rise.The cost is 45.31%,38.68%,5.97%,5.18%respectively,and the cost of medical expenses,medical expenses,medical expenses,nursing expenses,operation expenses,other expenses and blood transfusion expenses are 45.31% 2.94%,1.41%,0.28%,0.18%,1.31% and 0.03% respectively.3.The structure of the cost changes is 29.65% and the structural change is 3.29%.This was negative changes which including the medical expenses,medical expenses,medical expenses,medical expenses,medical expenses,medical expenses,transportation costs,transportation costs,blood transfusion fees,bed fees,nursing fees,medical expenses and others costs.From 2006 to 2015,the inspection fee,medical materials and medical expenses are the main factors which leading to the change of hospitalization cost structure.The cumulative contribution rate of them is 78.15%.4.The cost of hospitalization for tuberculosis patients Univariate analysis showed that hospitalization costs in age,residence location,payment,admission route,admission illness,surgery,the first visit,the year a total of eight different factors difference was statistically significant?P <0.05?,but there was no significant difference in the cost of hospitalization between tuberculosis patients?P> 0.05?.5.The main factors influencing the hospitalization expenses of tuberculosis patients were: drug charges,admission methods,hospital days,operation,the first treatment situation,payment method,P <0.05.The multiple linear regression equation is:Y=8.636-0.004X₂-0.015X₃-0.279X₄-0.086X₆-0.008X₇+0.119X₈+0.287X₉.Conclusion: 1.The age of tuberculosis patients is mainly concentrated in the middle age and young people who are caused a heavy burden to society and should focus on the young people,especially in school students.2.The average annual hospitalization cost of tuberculosis patients increased year by year,the average annual growth rate was 7.84%,the expenses were also increasing,and the economic burden of the disease was too heavy.3.The proportion of medical expenses and inspection fees is too high which are45.31% and 38.68% respective;treatment fees,nursing fees,surgical costs and other technical costs account a lower proportion,lower than 10%;inspection fees is Leading to a rise in the cost of hospitalization structure of the main factors.4.The main factors influencing the hospitalization expenses of tuberculosis patients were as follows: drug charges,admission methods,hospital days,operation,the first treatment situation,the payment method.
