Objective:CD31,CD34 antibodies were developed for the determinations of benign and malignant thyroid tumors' microvascular densities and compared with the pathological results.Then compare different marking methods of the difference of thyroid tumors,MVD;With the corresponding sonographic findings contrast,compare different marking methods for determining the results of thyroid tumors' MVD and the correlation of sonographic findings.Methods:1.The research objects:Collect 80 patients receiving sonographic exams and surgical treatments who suffered thyroid tumors in Southeast university affiliated Zhongda hospital from January 2013 to October 2013.There are 23 cases of male and female 57 cases,17 years old to 77 years old,the median age of 42.This study collected the benign thyroid tumors of thyroid adenoma,ruled out the nodular goiter and other tumor-like diseases.Collection of malignant thyroid tumors mainly thyroid cancers,including other parts of the tumor metastasis to the thyroid gland.51 cases of physical examination for the first time found lumps,no obvious clinical symptoms.29 patients due to touch the neck lump from days to months or sudden increase because of neck lump,5 cases with difficulty swallowing,another 1 cases retrosternal thyroid associated with breathing difficulties.According to postoperative pathological diagnosis can be divided into benign and malignant groups,most cases of preoperative fine needle biopsy,all specimens by immunohistochemical staining examination,have complete clinical and pathological and ultrasound datas.2.The inspection equipments and laboratory reagent2.1 By 2 work more than 5 years of ultrasound doctors use GE LOGIQ-S6 color doppler imaging device linear array probe frequency of 7.5?12.0 MHz(1 case of retrosternal thyroid using 3.5 MHz convex array probe),to judging the neck area of thyroid scan,and record the datas.For differences or cannot identify diagnosis of thyroid tumor nature to a supervisor,director of the physician when judging,ultrasonic conclusions.2.2 The Fuzhou Wallace new biotechnology development company's CD31,CD34 monoclonal antibodies and S?P kit.3.Observe the contents3.1 Use ultrasonic probe scan thyroid tumors tissues preoperative to observe the sonographic features of tumors and blood flow signals.Reference proposed level such as Adler's[1]classifications.3.2 Do immunohistochemical analysises after operatives by CD31,CD34 monoclonal antibodies labeling microvascular density.Observe the yellow,tan particles'distributions and countings reference to Weidner[2].Observe the differences between benign and malignant thyroid tumor MVD expressions.Compare the differences between CD31 and CD34 antibodies labeling microvascular densityies3.3 Observe whether there are relationships between CD31,CD34 antibody labeling the tumor's microvascular densities and thyroid tumors' two-dimensional features.3.4 Observe the correlations between thyroid tumors' blood flow signals classifications and MVD with CD31,MVD with CD34.Compare the relations between thyroid tumor blood flow signal classifications and MVD with CD31,CD34,and observe which one is more closely.4.Statistical analysisThyroid ultrasound characteristics of two-dimensional binary classification datas and pathological results use chi-square analysis.Intemal echo and pathological results of thyroid tumors use rank and test analysis.Comparisons between MVD-CD31 and MVD-CD34 use matching t test.The comparions between sonographic findings and thyroid tumors with CD31,CD34 antibodies labeling MVD datas use independent samples t-test analysis;Internal different echo of thyroid tumors,MVD compared with rank and inspection.Blood flow signals of different levels of thyroid tumors and classifications of MVD-CD31 and MVD-CD34 check data use Spearman correlation analysis;P<0.05,believe that there is a significant statistically,P<0.001,significant meaning.5.Results5.1 CD31 MVD marked 96 nodules,using independent sample t test shows,P<0.05,benign and malignant groups have differences.CD34 MVD marked 96 nodules,using independent sample t test shows,P<0.001,benign and malignant groups had significant difference.5.2 According to the ultrasonic two-dimensional ultrasonographic classifications,aspect ratio ? 1 or<1 tumor of CD31 expressions have no difference,P>0.05.CD31 expressions between the microcalcification classification of cancer have no difference,P>0.05).5.3 According to the shape,border,acoustic halo,internal echo,echo homogeneity classification of CD31 expressions between tumors have differences,P<0.05.According to CDFI classifiedMVD-CD31 have significant differences,P<0.001.5.4 Aspect ratio ?1 or the aspect ratio<1 MVD-CD34's expressions are no different between tumors,P>0.05.According to the shape,border,the echo homogeneity,microcalcification,the classifications of CD34 expressions between tumors have differences,P<0.05.According to the acoustic halo,internal echo and classifications of CDFI,MVD-CD34 are significantly different between tumors,P<0.001.5.5 CDFI classifications have significant correlations with CD31 expressions,P<0.001;CDFI have significant correlations with the expressions of CD34,P<0.001.Conclusion:The use of CD31,CD34 antibody markers of thyroid tumors'microvascular densities are higher than those in thyroid adenomas,and marking effect CD34 antibody is better than that of CD31 antibody.There are correlationships between two-dimensional ultrasounic features of benign and malignant thyroid tumors(except aspect ratio)with MVD.CDFI and CD31,CD34 antibodies labeling microvascular densities have correlation,and CD34 antibody markers related more closely.
