Background: With the improvement of people’s living standard and the change of diet,the incidence of diabetes is increasing year by year.Because the early symptoms of diabetes mellitus is not obvious,it is easy to be ignored for a long time.But in recent years,more and more attention has been paid to the effects of diabetic complications including diabetic nephropathy(DN),which is one of the most serious and harmful chronic complications caused by diabetes mellitus.Meanwhile,DN is also the cause of end-stage renal disease。Semaphorin3A,a member of the Semaphorins family,is a secreted glycoprotein that plays an important role in the development of diabetic nephropathy and becomes a new research hotspot at home and abroad,but the specific mechanism in diabetic nephropathy is not yet fully aware.Objective: To investigate the function of Semaphorin3A(Sema3A)in the pathogenesis of diabetic nephropathy(DN)through analyzing the expression level of Sema3 A in the serum of the patients with DN and provide a new theoretical basis for the prevention.Methods: 1.Selected 100 Type 2 diabetic patients and 20 healthy subjects who were hospitalized from department of Endocrinology,during March 2016 to Novembe 2016.2.Total 100 patients with type 2 diabetes mellitus were divided into simple diabetes mellitus(DM)group(n =30)and diabetic nephropathy group(n = 70).Diabetic nephropathy group was divided into 40 cases of early diabetic nephropathy(EDN)group and 30 cases of clinical diabetic nephropathy(DN)group.In addition,20 healthy persons were selected as healthy control group(NC).3.Counted general information of statistical research objects: age,duration of diabetes,measurement of height,weight,calculation of body mass index(BMI),random admission blood pressure.4.Determination of clinical parameters fasting blood sugar,glycosylated hemoglobin(Hb A1c),total cholesterol,triglycerides,creatinine,24 hour urinary albumin(m ALB),glomerular filtration rate(GFR).To investigate the correlation between various clinical index and diabetic nephropathy through statistical analysis of the above data.Result: 1.Compared with healthy people,the body mass index,blood pressure,blood lipid,fasting blood glucose and glycosylated hemoglobin were higher,and the difference was statistically significant(P < 0.05).Multivariate Logistic regression showed that SBP、DBP、TC、TG were associated with DN(P < 0.05)。 2.The level of serum Sema3 A increased gradually in the healthy control group,the simple diabetes group,the early diabetic nephropathy group and the clinical diabetic nephropathy group(P < 0.05),and Sema3 A in the serum was positively correlated with 24 hour urine microalbumin(m ALB)(P < 0.01).Conclusion: 1.Blood pressure,blood lipid and Hb A1 c are influence factors of diabetic nephropathy which is caused by many factors.2.The serum Sema3 A level in patients with diabetic nephropathy is positively correlated with urinary protein excretion rate and can be used to assess the type 2 diabetic renal damage and prognosis.
