ObjectivesThe rupture of intracranial aneurysm is with high morbidity and mortality, therefore the early intervention of unruptured intracranial aneurysms have positive significance. With a variety of inspection methods and the popularity, the unruptured intracranial aneurysm detection rate is increasing year by year,After the clinical evaluation of the risk of rupture of unruptured intracranial aneurysms,we tend to take the endovascular treatment. There are a few reports on the clinical endovascular interventional treatment in our country,so the safety and efficacy of endovascular interventional treatment for unruptured intracranial aneurysms are still needed to be further studied. This study retrospectively those who have accepted endovascular treatment of intracranial aneurysms in patients without clinical data of rupture of intracranial unruptured aneurysms interventional treatment indications, and analyzes the unruptured intracranial safety and efficacy of interventional treatment of the aneurysm.and at the same time,we collect and analysis data of TCM Syndromes of unruptured intracranial aneurysm patients,to explore the distribution of TCM Syndromes of unruptured intracranial aneurysms in patients with perioperative characteristics, provide a reference for the combination of traditional Chinese and Western medicine treatment for aneurysmal work.MethodsWe collect the clinical data of patients with unruptured aneurysms,of unruptured intracranial aneurysms in patients with sex, age, history of hypertension, smoking history, the size of intracranial aneurysm, interventional surgery, parts of the aneurysm filling degree, incidence clinical complications,follow-up related statistical analysis between February 6,2010-February 6,2016 in encephalopathy five department of the Hospital of Guangdong Province Traditional Chinese Medicine.Using SPSS17.0 software for statistical analysis, statistical methods using x2 test,t test through statistical analysis of efficacy and safety of interventional treatment for comprehensive evaluation of the patient, and the aneurysms in perioperative patients with TCM syndrome distribution characteristics were analyzed.ResultsIn this group there were 48 cases of unruptured intracranial aneurysm patients,a total of 59 unruptured aneurysms, including 26 cases were male(54.2%),22 cases were female(45.8%),the proportion of male and female is 1.2:1;the age of male is fluctuated in 41-82 years;the age of female is fluctuated in 28-74 year, the average age of men and women together are 57.3 years;the number of patients with hypertention are 23 cases, and non hypertensive patients are 25 cases;and 20 c patients are with smoking, while non-smoking patients are 28 cases; There are 48 small aneurysms,9 Medium-sized aneurysms,1 large aneurysms, and 1 giant aneurysm. There anterior 52 cases in circulation aneurysm,2 cases in the anterior cerebral artery aneurysm,4 cases in middle cerebral artery aneurysm,6 cases in anterior communicating artery aneurysm, the petrous segment of internal carotid artery 1 a cavernous segment of 4 aneurysms, clinoid segment 2,33 cases in supraclinoidal segment,4 cases in posterior circulation aneurysms,in which the 3 cases in posterior cerebral artery,3 cases in vertebral artery and 3 cases in the other parts,on embolization and grading, coil embolization in 32 cases, stent assisted in 27 cases;48 cases are of dense embolism,3 cases of residual aneurysm neck and 8 cases of partial embolization;perioperative complications occurred in 6 cases, the incidence was 10.1%,thrombosis operation in 5 cases,3 cases of stent thrombosis,2 cases of stent thrombosis,1 patients with contralateral cerebral angiography and the formation of internal carotid artery clinoid segment dissection with stenosis,1 cases of disability, disability rate was 1.6%:we followed up from 1-28 months after surgery with DSA or CTA,with an average of 7.4 months. There were 32 patients with a total of 41 aneurysms(69.5%).The results showed that 36 aneurysms(87.8%)embolization in stable condition,5(7.3%)of aneurysm recurrence,1 cases underwent re embolization,1 cases refused treatment.Patients with a history of hypertension, smoking history, the size of the aneurysm, aneurysm site, interventional therapy, had no statistical significance of unruptured aneurysm recurrence(P>0.05).Through the analysis of TCM syndrome, the syndrome distribution of patients with unruptured aneurysms in wind syndrome, phlegm syndrome, blood stasis and Qi deficiency, endovascular surgery for two before and after the operation of symptom change has little effect.Conclusions1.In this group of patients, the recurrence rate of the disability is low,the thrombotic events ocurred in the stent treatment, finally achieved good clinical outcomes, did not cause permanent nerve function defect. All of these tell us that the endovascular treatment of intracranial aneurysms is safe and effective for experienced interventional physicians;2.For the embolization of intracranial aneurysms were retrospectively studied,in patients with a history of hypertension, smoking history, the size of the aneurysm, aneurysm site, interventional therapy, had no significant effect on unruptured aneurysm recurrence, interventional therapy and embolism degree form does not have significant effect on the thrombus;3.For the purposes of TCM syndromes, the patients with TCM syndrome distribution of wind syndrome, blood stasis syndrome, phlegm syndrome, before and after the Surgery score did not decrease obviously, consider surgery for unruptured intracranial aneurysms in the perioperative period has little effect of TCM Syndrome.
