ObjectiveThis study starts with the syndrome differentiation of Yin and Yang Syndromes of acute ischemic stroke,with MMSE and MoCA scale as the research tool,objective to explore the difference of cognitive dysfunction and cognitive domains between Yin and Yang syndromes of acute ischemic stroke.The dynamic process of cognitive function after acute ischemic stroke was observed,so as to understand the tendency of cognitive decline of Yin Syndrome and Yang type syndrome after acute ischemic stroke,and we should find a new way to improve cognitive dysfunction after acute ischemic stroke.MethodsA total of inpatients of the Five with acute cerebral infarction in Zhuhai Hospital of Guangdong Province Traditional Chinese Medical Hospital from May 2016 to March 2017 were collected.On the day of admission,the syndrome was divided into Yin and Yang syndrome groups,and related data were collected according to inclusion/exclusion criteria.The Mini-Mental state examine,the Montreal cognitive rating scale,the Hamilton depression scale and Hamilton anxiety scale were used to assess the cognitive status within 14 days after admission.If the patients was HMDA≥8 points and HAMA>7points,those would be excluded,all of the rest patients enrolled in the study were followed up.At the end of the 3 months,we would call the patients and asked them to go back to the hospital for a second evaluation.Finally,patients completing the two evaluation would be included.In the end,62 cases were collected,30 cases of Yin type syndrome,32 cases of Yang type syndrome.Recording the general demographic,including age,gender,education,hypertension,diabetes,atrial fibrillation,smoking,drinking,imaging data,NIHSS score,etc.Second days after admission,the venous blood 2-3ml was taken at an early morning,Included glycosylated hemoglobin(HbA1C),triglyceride(TG),cholesterol(TC),low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C),and homocysteine(HCY).Results1,The MMSE scale showed that the incidence of vascular cognitive impairment was 64.5%after 3 months of cerebral infarction,including 21 cases of Yin Syndrome group(52.5%),and 19 cases of Yang syndrome group(47.5%).2,The datas of patients of Yin Syndrome and Yang syndrome group,include glycosylated hemoglobin(HbA1C),triglyceride(TG),cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),homocysteine(HCY),age,gender,education,hypertension,diabetes,atrial fibrillation,smoking,drinking,the number and location of cerebral infarction,NIHSS score and within 14 days of the scores of the MMSE and MoCA scales,the difference of those was not statistically significant(P>0.05).3,The total scores of MMSE and MoCA scales in the two groups at the end of 3 months were showed that the scores of the Yin Syndrome group were slightly lower than that of the Yang type group,but there was no statistical difference between the two groups(P>0.05).Further,it was showed that at the end of the 3 months of the scores of the MMSE and MoCA scale of the two groups of patients were compared,(1)in the MMSE scale,The scores of the time directed force of Yin group were significantly lower than those of the positive group,it was statistically significant difference between two groups(P<0.05);(2)in the MoCA cognitive assessment scale,the scores of visual space and executive ability,naming ability in the Yin group were significantly lower than those in the positive group,and the difference was statistically significant(P<0.05).4,The changes dynamically of within 14 days of the sores of MMSE and MoCA scales compared with at the end of 3 months of those was showed:(1)In the Yin syndrome group:①although the total scores of MMSE and MoCA in the patients was lower than that in the 14 day,the two groups had no statistically significant difference(P>0.05).②According to the subitem of MoCA scale,The delayed memory ability at the end of 3 months was significantly lower than that in the 14 day,and the difference between the two groups was statistically significant(P<0.05);(2)In the Yang syndrome group:①he total scores of MMSE and MoCA scales in the patients was lower than that in the 14 day,but the difference was not statistically significant(P>0.05);②At the end of 3 months the difference in recall ability of the subitems of MMSE and MoCA scales compared to the 14 days’ s was statistically significantly(P<0.05).Conclusion1.Cognitive impairment may be more serious in patients with Yin deficiency syndrome after acute ischemic stroke:①At the end of 3 months,both the MMSE scale and the MoCA scale showed that the scores in the Yin class group were lower than those in the Yang Group,mainly in short-term memory,attention and computation,delayed recall,executive and spatial structure.②At the end of 3 months,the MoCA scale showed that the visual and executive ability decreased significantly in the cognitive field.③The MMSE and MoCA scales showed that most cognitive areas in the two groups decreased at the end of3 months compared with the 14 day,were slightly more severe in the Yin class.2,The decrease of delayed recall was the main manifestation of cognitive impairment after acute ischemic stroke:The MoCA scale showed that at the end of 3 months,the cognitive impairment in the two groups was delayed mainly compared with the 14 day period.
