Clinical Evaluation Of LNG-IUS For Treatment Of Adenomyosis

Objective:To investigate the Clinical efficacy and safety evaluation of levonorgestrel-relesing intrautenine system(LNG-IUS)for treatment of adenomyosis.Methods:All 102 patients,who ask to put LNG-IUS,were presented in the period from October 2013 to December at Women's Hospital School of Medicine Zhejiang University and were diagnosed with adenomyosis.Among them,19 cases received the treatment of lesion excision and/or GnRHa needle in the first six months before LNG-IUS.They were retrospectively analyzed and follow-up at the time of 3?6?12?36 months postoperativly to evaluate menstruation,dysmenorrhea NRS,PBAC,wearing time,and adverse reaction before and after treatment.Compare the Clinical efficacy and safety between pretreated patients and matched patients.Results:61.8%of 102 cases were not taking ring(follow-up time>36 months),dysmenorrhea score of NRS before put LNG-IUS is 6.03±1.95,after treatment is 1.78 ±2.30,dysmenorrhea symptoms relieved a lot(P<0.01),all of them feel much better at the first 3 months.Menstrual volume PBAC:88.2%cases reduced significantly,PBAC before LNG-IUS is 103.58±23.35,after LNG-IUS is 26.92±31.80(P<0.01),stabilization time of menstrual were 40.2%?72.5%?79.4%at3?6?12?36 months.The main adverse reactions:amenorrhoea 7.8%,shedding automatic 14.7%,oligomenorrhea 20.6%,continuous vaginal bleeding 13.7%,no significant relief of 5.9%,completely ineffective in 4.9%.Pretreatment and less menstrual volume can extend the wearing time.Conclusion:LNG-IUS can control the clinical symptoms of patients with adenomyosis,effectively alleviate the symptoms of dysmenorrhea,and significantly reduce the amount of menstruation,it can effectively control the process of adenomyosis,reduce side effects.Patients received the treatment of lesion excision and/or GnRHa needle in the first six months before put LNG-IUS have high rate of wearing.
