Objective:To explore the effects of terlipressin used in Patients with Hepatocellular Carcinoma combined with Hepatitis B Cirrhosis after hepatectomy.We did this survey to find an effective way to reduce incidence of complications and speed up recovery after surgery.Methods:From May 2016 to November 2016,we collected 100 patients with hepatocellular carcinoma combined with hepatitis B cirrhosis who accepted hepatectomy in Shandong Provincal Hospital,and randomly divided patients into experimental group and control group.Each group had 50 patients.And there is no obvious difference in the the sex,age,Hb,PT,T-bil,surgery time,amount of bleeding and time of clamping porta hepatis between two groups.The patients in experimental group(terlipressin group)were continuously pumped terlipressin(2 mg/d,dissolved by 40 ml 0.9%sodium chloride solution)from the day immediately after surgery,and the treatment should continue no more than 12 days.The traditional method-diuretic and Dopamine were applied in control group(no terlipressin group).T-bil,ALT,AST,Cr and BUN value were recorded respectively in the first,third and fifth days after surgery,extubation time,cases of pleural effusion and hospital stays were also collected in this study.The status were analyzed by SAS software.Result:The value of T-bil,ALT,AST,Cr and BUN of experimental group of recorded in the first day after surgery were 30.8023 ± 3.06435 umoll/L?370.51±43.762 U/L?482.14±48.126 U/L?65.240±3.4698 umol/L.6.24±0.399 mmol/L,while the value in control group were 36.8396±5.41013 umol/L?433.54±55.070 U/L?483.22±54.129 U/L?66.190±3.2626 umol/L?6.35±0.394 mmol/L,there is no statistical significance between the two groups(p>0.05).The value of T-bil,ALT,AST,Cr and BUN in experimental group recorded in the third day after surgery were 39.523±3.4553 umol/L?97.66±15.5U/L?205.30±28.317 U/L?61.94±2.855 umol/L?6.933 ±0.4423 mmol/L,value in control group were 54.207 ±5.7861umol/L?150.16±21.4U/L?261.84±43.882U/L?71.25±3.350umol/L?7.430± 0.3138mmol/L,there is statistical significance between the values of indexs in two groups(p= 0.0027,0.0439,0.0454,0.0366,0.0365);The value of T-bil,ALT,AST,Cr and BUN in experimental group recorded in the fifth day after surgery were 33.284±4.0263umol/L±59.00±8.827U/L?86.73±12.702U/L?62.23±2.084umol/L?5.943±0.3276 mmol/L,value in control group were 54.963 ± 6.8413umol/L?69.47 ±6.225U/L?122.17±24.010U/L?69.52±3.747umol/L.7.186±0.4525mmol/L,there is statistical significance between the values of indexs in two groups(p=0.0001,0.0002,0.0210,0.0196,0.0317).The extubation time of experimental group(7.31 ±0.554 days)was shorter than that of control group(8.25 ±0.667 days)(p=0.034);The incidence of pleural effusion in experimental group were significantly fewer than that in control group,P=0.045;The duration of hospitalization in experimental group(10.03 ± 0.256days)was shorter than that in control group(11.23 ± 0.335 days)(p=0.041).Conclusion:The application of terlipressin in patients with hepatocellular carcinoma combined with hepatitis B cirrhosis after hepatectomy can improve the function of liver and kidney after surgery,and shorten the extubation time,the duration of hospitalization and reduce incidence of pleural effusion.As a result,for patients with hepatocellular carcinoma combined with hepatitis B cirrhosis after hepatectomy,terlipressin can be used to reduce incidence of complications and speed up recovery after surgery.
