Objective:The hypnotic agent propofol has pharmacokinetic characteristics that allow for rapid onset and offset of drug effect and fast elimination from the body,and wildly used in anesthesia.However,patients with chronic alcoholism are frequently presented for anaesthesia and surgery,and it is commonly accepted that chronic alcoholism is associated with increased doses of anaesthetic agents.This study was designed to investigate the possible mechanism of chronic alcohol intake on propofol.Methods:Twenty four male patients undergoing gastrointestinal surgery were assigned into two groups by the ADULT scale,each 12 person.General anesthesia was induced by propofol TCI.The hemodynamic changes during surgery were recorded.Blood samples were collected on specific time during surgery and after propofol TCI.The concentration of profofol in plasma was detected by HPLC-FLD.Results:HPLC-FLD method:calibration curve was y = 0.5622x-0.0017(R2 = 0.9999),linear range covers 0.1?5?g/mL,recovery rate of propofol was between 95%?105%,and the endogenous substances in the blood and concomitant medications did not interfere with the determination.The hemodynamic changes in alcoholism group were more than control group(p<0.1),and the steady state plasma concentrations(Css)of propofol was no significant differences between the two groups,however the propofol concentrations after propofol TCI in alcoholism group were higher than that in control group(p<0.1).Conclutions:Chronic alcoholism does have a certain influence on propofol anesthesia.There are cross tolerance between ethanol and propofol,but it doesn't due to the pharmacokinetic differences completely.
