objective: Bone marrow microenvironment is closely related to the growth and differentiation of normal hematopoietic cells,as well as the proliferation and metastasis of blood tumor cells.Multiple myeloma(MM)of the tumor cells because of the close relationship between bone marrow hematopoietic microenvironment and settled in the bone marrow and bone marrow infiltration and few organs outside,and the bone marrow stromal cells and cytokines are closely related.MM due to the low proportion of myeloma cells,high resistance,easy to relapse,resulting in clinical treatment difficulties,can not be cured,is still a progressive,fatal disease.At present,the pathogenesis,treatment and drug resistance of MM still need a lot of research work.Due to the development of cytology,immunology,molecular biology and bone marrow environment research and application of the emblem,at present there have been some new molecular biology based drugs targeting angiogenesis,with targeted therapy drugs are more mature in these drugs such as thalidomide and lenalidomide.But the specific mechanism of anti tumor drugs that are not fully understood.Therefore,the mechanism of angiogenesis is worthy of further exploration,especially in the areas of tumor disease,ischemia and hypoxia,and inflammatory diseases.Microvessel density(MVD)is the most commonly used index of angiogenesis in response to angiogenesis.So the design of this experiment is to: 1.To explore the differences in expression of MM and normal bone marrow in SDF-1/CXCR4 axis and clinical indicators of MM disease and correlation;2.To explore the expression of MVD in bone marrow MM and normal bone marrow,the difference of MM and its clinical index and correlation;3.To explore the correlation between MM CXCR4,MVD and SDF-1 in bone marrow;4.Analyze the clinical significance of SDF-1/CXCR4 axis in multiple myeloma patients,to provide a new way to explore the pathogenesis of multiple myeloma and find effective treatment methods.Methods : 1.The Affiliated Hospital of Medical College of Southwest collection of Hematology,2013 01 to 2016 06 months between newly diagnosed multiple myeloma patients biopsy specimens were obtained from 63 cases as experimental samples,20 normal subjects as control specimens by immunohistochemical staining method was used to detect the expression of multiple myeloma in experimental group and normal control group SDF-1,CXCR4,MVD.2.The clinical data of sex,age,clinical stage,immunoglobulin and light chain immunity in the experimental group and control group were collected.3.The above datas will be analysised by statistical.Results: 1.The expression of SDF-1 in the experimental group was significantly higher than that in the control group,which was statistically significant(P=0.000<0.05).Newly diagnosed multiple myeloma patients SDF-1 expression with sex and age of patients,the immunoglobulin light chain type,classification showed no difference(P>0.05),but there was significant difference between and the patients with ISS stage(P=0.000<0.05).2.The expression of CXCR4 in the experimental group was significantly higher than that in the control group,which was statistically significant(P=0.000<0.05).The expression of CXCR4 in bone marrow of patients with newly diagnosed MM was not significantly different from that of patients with gender,age,immunoglobulin type and light chain classification(P>0.05),but there was significant difference between and patients with ISS stage(P=0.002<0.05).3.The expression of MVD in bone marrow of experimental group was significantly higher than the control group,the expression of MVD in bone marrow of patients with newly diagnosed MM with sex and age of patients,the immunoglobulin light chain type,classification,the difference was not statistically significant(P>0.05),and the MM staging of the disease had statistical difference(P=0.000<0.05),The results of correlation analysis showed that the expression of MVD in bone marrow of MM was positively correlated with ISS stage(rs =0.781,P=0.000).4.Correlation analysis showed that the expression of SDF-1 was positively correlated with the expression of CXCR4 and MVD in the bone marrow of MM(rs=0.478,P=0.000;rs =0.437,P=0.000).Conclusion: 1.The expression of SDF-1/CXCR4 in the bone marrow of newly diagnosed MM patients was significantly higher than that of the patients;2.The expression of MVD in the bone marrow of newly diagnosed MM patients was increased,and was positively correlated with MM stage,suggesting that MVD could be used as an indicator of disease progression and prognosis;3.The expression of SDF-1/CXCR4 in the bone marrow of patients with newly diagnosed MM was positively correlated with the expression of MVD,suggesting that SDF-1/CXCR4 biological axis can promote the angiogenesis of MM.4.The SDF-1/CXCR4 biological axis can be used as the target of MM angiogenesis therapy.
